基于我国现有的医保制度,利用动态最优控制模型进行优化分析,通过对公立医院利益的分析,构建公立医院与患者的利益补偿均衡模型,试图从理论上分析公立医院与患者之间的利益补偿均衡问题。以公立医院与患者之间最小单位风险收益差作为双方就医谈判的目标函数,并通过多次迭代得到最优解P2*和Q2*。通过对补偿机制的改革以及药品价格机制的调整,公立医院收益的增加会使其参与合约交易的积极性增加,差价合约能有效规避诊疗价格的波动给医患双方带来的风险。
Based on current health care system in China, we make an optimization analysis by dynamic optimal control model. Through the benefit analysis of public hospitals, we construct the benefit compensation equilibrium model of public hospitals and patients in an attempt to analyze theoretically the benefit compensation balance between public hospitals and patients. With the smallest unit risk-income difference as the objective function of the medical negotiation between public hospitals and patients, we get the optimal solutions of P2* and Q2* by iteration in the function. Through the reform of the compensation mechanism and the adjustment of drug price mechanism, the income increase of public hospitals will promote the contract transaction enthusiasm. Contract for difference can effectively help to evade the risk of doctors and patients due to price fluctuation.
