目的:探讨人肝细胞肝癌组织中血小板衍生生长因子受体α(PDGFRα)表达及其与预后的关系。方法:采用免疫组化法检测57例石蜡切片标本中癌组织及癌旁正常肝组织 PDGFRα的表达,另外14例新鲜标本则由 Western blot 法检测。进一步分析其表达与肝癌患者临床病理特征、预后及肿瘤微血管密度间的关系。结果:57例肝癌组织中22例 PDGFRα高表达(38.6%),而57例癌旁正常组织中仅4例高表达(7%)。PDGFRα表达与肝硬化、瘤体大小、数量、分化程度及甲胎蛋白水平无显著相关性(P >0.05),但其表达与血管侵犯却显著相关(P =0.010)。Spearman 相关分析还显示,PDGFRα的高表达与血管侵犯之间正相关(r =0.212,P <0.001)。22例 PDGFRα高表达患者肝癌组织中微血管密度(MVD)较低表达者显著升高[(43.3±4.4)/0.74mm2 vs.(29.2±1.9)/0.74mm2,P =0.024]。PDGFRα高表达患者总体生存率(OS)和无瘤生存率(DFS)较低表达患者显著降低(对数秩检验,P =0.005和 P =0.025)。结论:肝细胞癌中 PDGFRα过表达导致预后变差可能是通过促进肝癌血管生成与侵犯实现的。
Objective:To investigate the expression of platelet - derived growth factor receptor α(PDGFRα)in hepatocellular carcinoma(HCC)and the association with prognosis of the patients after curative hepatectomy. Meth-ods:The expression of PDGFRα in HCC and adjacent normal liver tissues from 57 parafin - blocked specimens and 14 fresh specimens were detected and compared by immunohistochemistry and Western blot. The correlations of their expression to the clinicopathologic characteristics,prognosis of the HCC patients and microvessel density(MVD)of the tumors were explored. Results:High protein level of PDGFRα was seen in 22 out of 57(38. 6% )HCC tissues, compared to only 4 out of 57(7. 0% )adjacent liver tissues. Although no significant correlation was observed between PDGFRα expression and liver cirrhosis,tumor size,number,differentiation or alpha - fetoprotein level(P 〉 0. 05), statistically significant correlation between PDGFRα expression and macroscopic vascular invasion(MVI)was found (P = 0. 010). Spearman analysis also revealed that high PDGFRα expression was positively correlated with vascular invasion(r = 0. 212,P 〈 0. 001). The MVD in 22 patients with high PDGFRα expression was significantly higher than those with low PDGFRα expression[(43. 3 ± 4. 4)/ 0. 74mm2 vs.(29. 2 ± 1. 9)/ 0. 74mm2 ,P = 0. 024]. The results showed patients with high PDGFRα expression had significantly worse overall survival(OS)and disease - free sur-vival(DFS)than those with low PDGFRα expression(log - rank test,P = 0. 005,and P = 0. 025). Conclusion:Overexpression of PDGFRα in HCC indicates poor prognosis in HCC probably via promoting HCC angiogenesis and MVI.
