目的:探讨金创抗菌膏对治疗糖尿病难愈性皮肤溃疡Ⅰ、Ⅲ胶原及MMP-13表达的影响。方法:32只糖尿病大鼠模型造模成功的雄性SD大鼠随机分为生理盐水组和金创抗菌膏治疗组(每组16只),另外选择16只健康雄性SD大鼠作为对照组,三组大鼠同时制作创面模型。创面造模成功后,每天每组分别换药,于第7 d、14 d,每组处死8只大鼠。HE染色观察压疮皮肤形态改变;免疫组织化学技术检测基质金属蛋白酶13(matrix metalloproteinase 13,MMP-13)的表达变化,RT-PCR技术检测MMP-13 m RNA的表达;苦味酸-天狼星红染色观察Ⅰ、Ⅲ型胶原面积变化。结果:HE染色显示金创膏组较生理盐水组更早出现肉芽组织,且成纤维细胞排列规则呈"指纹状";对照组成纤维细胞极性消失及胶原纤维排列紊乱,肉芽组织出现较晚。免疫组化及RT-PCR显示,金创抗菌膏治疗组MMP-13表达比对照组和生理盐水组均降低(P〈0.05)。苦味酸-天狼星红染色显示与其它两组比较,金创抗菌膏组Ⅲ胶原面积最大,Ⅰ型胶原面积最小。结论:金创抗菌膏可以促进大鼠糖尿病皮肤溃疡的愈合,可能与调控创面皮肤Ⅰ、Ⅲ胶原基因及MMP-13的表达相关。
Objective: To investigate the curative effects of antiseptic ointment in a rat model on diabetic ulcer and to explore its potential mechanism. Methods: 32 rats, after establishing the model of diabetic ulcer,were divided into 2 groups randomly: physiological saline group and antiseptic ointment treatment group, and 16 healthy rats were used as a control group. The changes of the morphological structure was examined by HE staining; the expression of MMP-13 was determined by immunohistochemical staining; the m RNA expression of MMP-13 was assessed by RT-PCR; and the area of collagen Ⅰand collagen Ⅲ were measured by picric acid- Sirius red staining. Results: Compared with control group, m RNA and protein expression of MMP-13 were decreased remarkably in antiseptic ointment treatment group(P〈0.05), and the area of collagen Ⅲ was increased. Conclusion: The expression of type Ⅰand Ⅲ collagens and matrix metalloproteinase 13 can be modulated by antiseptic ointment in a rat model of diabetic skin ulcer.
