髋臼前柱顺行拉力螺钉固定钉道的数字化分析与验证

目的通过数字化分析、优化髋臼前柱顺行螺钉钉道,建立个性化髋臼前柱顺行螺钉固定的最优钉道。方法选取40名健康成年志愿者,男20名,女20名;年龄为18~56岁,平均34.6岁。腹部行连续螺旋CT扫描,电压为120kV,层厚为0.625mm,矩阵为512×512。将原始Dicom格式数据导入Materialise Mimics Innovation Suite16.0软件,三维重建骨盆,以STL文件格式存储,导入Imageware12.0软件,确定最优钉道,测量其直径和长度,并比较不同性别、不同侧别之间的差异。取干燥成人髋骨标本20具,行连续螺旋CT扫描,同法确定最优螺钉通道,并制作拉力螺钉固定导航模板,顺行固定髋臼前柱后行影像学评估。结果髋臼前柱安全区投影呈不规则三角形,男性投影面积显著大于女性,差异有统计学意义(P〈0.05)。男性左侧髋臼前柱可置入拉力螺钉的最大直径及最大长度平均分别为(7.12±0.39)、(109.99±2.89)Film,右侧平均分别为(7.04±0.34)、(110.11±2.43)mln;女性左侧平均分别为(6.34±0.38)、(94.75±2.02)mm,右侧平均分别为(6.36±0.30)、(94.73±1.51)mm。同一性别的左、右侧之间比较差异均无统计学意义(P〉0.05),而同一侧别男、女性之间比较差异均有统计学意义(P〈0.05)。所有经模板打入的克氏针均无穿透骨皮质或侵入髋关节。结论髋臼前柱确实存在一可行拉力螺钉内固定的骨性通道,基于CT图像的数字化分析能够确定髋臼前柱顺行螺钉的钉道及安全区,借助个体化导航模板可以使置钉过程更为安全、有效。

英文摘要:

Objective To determine a safe tract for placement of anterograde lag screws in indi- vidualized fixation of the acetabular anterior column by digital analysis and optimization. Methods We recruited 40 healthy adult volunteers in this study. They were 20 men and 20 women, 18 to 56 years of age (average, 34. 6 years). They underwent continuous spiral CT scanning on the abdomen, with voltage of 120 kV, slice thickness of 0. 625 mm and matrix of 512 ×512. Firstly, CT scanning images from the 40 healthy adult volunteers were transformed into Materialise Mimics Innovation Suite 16. 0 software to obtain 3D recon- struction images of the pelvis (in . stl files). Then the Imageware 12. 0 software (EDS, USA) was used to produce optimal and safe tracts for placement of anterograde lag screws in fixation of acetabalar anterior col- umn. The diameters and lengths of the screw tracts were measured and compared between genders and sides. Secondly, 20 dry specimens of adult hip were scanned. After the optimal screw tracts were determined and measured in the same way, an individualized navigation template for anterograde screw placement in fixation of acetabular anterior column was generated. After fixation of the anterior column of the acetabulum was con- ducted using Kirschner wire with the aid of navigation template, imaging evaluation was performed. Results The projection of acetabular anterior column was like an irregular triangle in shape. The projection sizes varied significantly between individuals and the male ones were significantly larger than the female ones ( P 〈 0.05). The maximum diameter and length of the lag screw for the male anterior column were 7. 12 ± 0. 39 mm and 109.99±2.89 mm at the left side, and 7.04±0.34 mm and 110. 11 ±2.43 mm at the right side; the maximum diameter and length of the lag screw for the female anterior column were 6.34 ± 0. 38 mm and 94. 75 ± 2.02 mm at the left side, and 6. 36 -e 0. 30 mm and 94. 73 ± 1.51 mm at the right side.
