兔膝单侧前交叉韧带损伤后双侧韧带内本体感受器变化的实验研究

目的:探讨前交叉韧带( ACL)部分损伤后双侧ACL内本体感受器的形态及功能变化及其机制。方法使用12只新西兰大白兔造单侧前交叉韧带部分损伤模型,4只新西兰大白兔作为对照组,模型组分别于术后2、4、6个月各取4只行体感诱发电位和腘绳肌肌电图检测,之后取前交叉韧带行氯化金染色检查,评估韧带内神经组织形态及功能情况。结果损伤侧随着时间延长,其体感诱发电位和肌电图的潜伏期逐渐延长和波幅逐渐下降, ACL内本体感受器数量逐渐减少,异形的本体感受器增多、体积缩小,并出现部分结构缺失,与对侧及对照组对比,均有统计学差异(P<0.05)。对侧体感诱发电位和肌电图的潜伏期与对照组对比并无统计学差异(P>0.05),ACL内本体感受器数量和形态无明显变化。结论 ACL损伤后其本体感受器出现数量减少、结构异常,但是在短期内依然能发挥一定功能,随损伤时间延长其作用逐渐减弱。而对侧膝关节似乎也有本体感觉下降趋势,但无统计学差异,仍需进一步研究。

英文摘要:

Objective To explore the shape and functional changes of the proprioceptors in the partly injured anterior cruciate ligament ( ACL) as well as the ones in the contralateral ACL of the rabbits . Methods 12 New Zealand white rabbits were in the group of partly injured ACL model , and four rabbits were in the control group.The two groups were examined by the somatosensory evoked potential (SEPs) and the electromyogram ( EMG) of the hamstrings ;the gold chlorid staining method was also performed to evaluate the shape and functional changes of the nerve structures in two , four, and six months post-operatively ( four rabbits were sacrificed at each time point ) .Results The SEPs and EMG's incubation period and the wave amplitude presented a slow drop along with the time .The differences in the incubation period and the wave amplitude between the model sides and the contralateral sides were statistically significant ( P 〈0.05 ) , and the differences between the model group and the control group were statistically significant (P〈0.05) too.The differences in the incubation period and the wave amplitude between the control group and the normal sides of the model group were not statistically significant ( P>0.05).The number of the proprioceptors in the injured ACL gradually reduced with the time ; the morphological abnormality , volume diminution and structure deficiency were also found in the proprioceptors .Yet there was no abnormal changes in the contralateral sides of the model group . Conclusions The proprioceptors can still restore some functions after the ACL injury ,but the functions were weakened languishingly as the time passing .There seems to be a downward trend of the contralateral proprioceptions of the lower limb , but there is no statistical significance .The further study is needed .
