目的:系统评价TCF7L2基因rs7903146多态性与2型糖尿病的关系。方法:通过中国知网(CNKI)和万方数据库及Pub Med全面检索公开发表的关于TCF7L2基因rs7903146多态性的研究,采用Stata 11.0软件对纳入的文献进行Meta分析。结果:最终纳入文献25篇,共纳入25 560例研究对象,糖尿病组13 748例,对照组11 812例。(TT+CT)/CC基因型Meta分析异质性为I~2=72.9%,所以采用随机效应模型合并数据,合并效应量(TT+CT)/CC基因型频率OR值为1.52,95%CI:1.34~1.71,(Z=6.75,P=0.000);东南亚、欧洲、美洲差异有统计学意义,西亚人群中差异无统计学意义;敏感性分析稳定,漏斗图提示不存在发表偏移。等位基因T/C Meta分析异质性为I~2=73.5%,采用随机效应模型;合并效应量等位基因T/C的OR值为1.43,95%CI:1.30~1.57(Z=7.39,P=0.000),各亚组之间差异有统计学意义;敏感性分析稳定,漏斗图提示无发表偏倚。结论:TCF7L2基因rs7903146多态性与2型糖尿病的发生有关,TT+CT基因型是T2DM患者发病的危险因素;等位基因T是T2DM患者发病的危险因素。
AIM:To systematically review the relationship between polymorphism of rs7903146 inTCF7L2 gene and 2-diabetes mellitus.METHODS:Researches about rs7903146 inTCF7L2 gene were selected from Chinese National Knowledge Infrastructure,Chinese Wanfang Data Knowledge Service Platform,and Pubmed database.Software Stata 11.0 was applied to do Meta-analysis about these researches.RESULTS:Twenty five studies were eligible for inclusion in the Meta-analysis.There was 25 560 cases in this study which included 13 748 patients in group T2DM and 11 812 healthy people in Control group.Meta-analysis results showed that literature of gentype(TT + TC)/CC had higher heterogeneity (I2 =72.9%).After combining analysis by using random effect model,the OR value of genotype frequency of effectsize (TT + TC)/CC was 1.52,(95% CI:1.34-1.71,P =0.000).Significant difference was detected among the population of Southeast Asia,Europe and America while was absent in West Asia.The results of Meta-analysis were stable without significant publication bias after sensitivity analysis and funnel plot of the Begg's test and Egger’s test.The random effect model was used to analysis allelic gen of T/C for the higher heterogeneity (I2 =73.5%).The results were stable and pooled OR 1.43,(95%CI:1.30-1.57,P=0.000).No publication bias was found in this study.CONCLUSION:There is significant correlation between polymorphism of rs7903146 in TCF7L2 gene and 2-diabetes mellitus.The genotype of TT +CT is a risk factor of the incidence of T2DM.The allelic gen of T is alsoa risk factor of T2DM.
