目的探讨神经细胞黏附分子(NCAM)在人隐睾中的病理表达及意义。方法 48例隐睾患儿,年龄6个月~13岁,平均3岁。单侧隐睾43例,双侧隐睾5例。共50个隐睾组织活检样本,应用免疫组化法检测NCAM在生精小管和间质细胞的表达。结果 58%(29/50)的标本NCAM生精小管阳性或间质细胞阴性病理表达。30%(15/50)的标本NCAM在生精小管阳性病理表达,不同年龄、睾丸位置的阳性表达率间差异无统计学意义(P〉0.05)。32%(16/50)的标本NCAM在间质细胞性病理表达,不同睾丸位置的阴性表达率间差异无统计学意义(P〉0.05),在不同年龄组间差异有统计学意义(P〈0.05)。结论 NCAM在隐睾中的病理性表达情况反映了生殖母细胞的迁移、分化障碍以及间质细胞分化延迟,可以作为预测隐睾生殖潜能的可靠分子标志物。
Objective To investigate the expression of Neural Cell Adhesion Molecule(NCAM)in the cryptorchidism and its clinical significance.Methods This study comprised 50 testis biopsies of 48 children(43 bilateral and 5 unilateral)who underwent orchiopexy between 6 months and 13 years,their mean age was 3 years.NCAM was detected by Immunohistochemistry in the seminiferous tubule and Leydig cell.Results 58% of the samples(29/50)was detected NCAM positive in the seminiferous tubule or negative in Leydig cell,which were both pathological expression.30% of the samples (15/50) detected positive in the seminiferous tubule had no relations with the location of the testis and the children age (P0.05).32% of the samples (16/50) detected negative in the Leydig cells had no relation with the location of the testis.There was significant differenceamong the different age groups,and the positive rate increased with the age.Conclusion The pathological expression of NCAM in cryptorchidism biopsy tissue reflected there was a defect or delay in the migration and differentiation of Gonocytes and Leydig cells.NCAM could be a new molecule marker for estimating the fertility potential and canceration in cryptorchidism.
