【目的】观察不同离体时间的非血管化游离骨块同期种植体植人体内后血管形成的情况,进而评价离体时间对非血管化游离骨移植后成活的影响。【方法】截除狗下颌骨的无牙颌骨段,取自体髂骨块。在离体10、20和80m内植入纯钛金属种植体,修复下颌骨的节段性缺损处,恢复下颌骨的连续性,分别在术后1、2、4周处死动物,取材前用墨汁行血管灌注;免疫组织化学方法行Ⅷ因子抗原染色血管标记。【结果】在游离骨移植术后1、2、4周的时间点,可见非血管化骨块内墨汁灌注的血管数逐渐增多,标志血管的内皮细胞由团块状,同心圆状,逐渐向中空的血管样演变,离体10min和20min组之间有所差别,但没有统计学的意义,P〉0.05;80min组墨汁灌注血管较少,Ⅷ因子抗原染色标记的血管出现较晚,计数较少,与离体10和20min组之间有显著的差异(P〈0.05)。【结论】非血管化游离骨块同期种植体修复颌骨节段性缺损时,游离骨块的离体时间是影响血管生成的主要因素,离体时间越短越有利于血管的生成和移植骨的成活。
[Objective] To investigate the angiogenesis of transplanted free nonvascularized bone graft (NBG) with dental implant in different off-body time. [Methods] The parts of edentulous jaw was exsected off, the mandible segmental defects space was restored respectively by free nonvascularized auto iliac grafts with pure titanium dental implants in 10 (A), 20 (B), 80 (C) minutes. Cervical vascular perfusion was done with Indian ink before animals executed, factor Ⅶ antigen staining was made to mark the vessel inside the transplanted bone graft after operation 1, 2, 4 weeks. [Results] The Indian ink perfusion marked vessels increased gradually in the period of 1 - 4 weeks after operation. The vessel marker, revealled that the endothelial cells transformed from mass or concentric mass to vessel-like hollow form. The vessel counts between group A and group B were not different, but they were much different from that of group C. The counts of Indian ink perfusion vessel were less and factorⅦ antigen marked vessel presented later in group C as compared to that of group A and B ( P 〈 0.05) [Conclusions] The off-body time of free NBG is one of the main factors of its angiogenesis in restoring segmental jaw defects by NBG with simultaneous dental implantation.
