双腔引流管治疗高血压性脑出血观察与创新研制

目的观察自制双腔引流管在治疗高血压性脑出血中的疗效,为研制一种专用、有效的颅内血肿微创治疗的新型引流装置提供理论基础。方法对2011年6月~2015年12月104例高血压性脑出血患者随机分为两组:对照组41例,用单腔脑室外引流管;治疗组63例,用自制双腔引流管,手术方式一致。观察两种引流管治疗后拔管时间、血肿清除效果、并发症及预后的效果。结果治疗组血肿清除率明显高于对照组(P〈0.01),5d内拔管率高于对照组(P〈0.05),对照组引流管堵塞、断裂、脱出风险高于对照组(P〈0.05);治疗组优级(ADLI级+ADLII级)高于对照组;治疗组死亡率与对照组比较差异无统计学意义(P〉0.05)。结论自制双腔引流管用于微创治疗高血压性脑出血,可直接引流颅内血肿,具有加强引流效果,注药简单方便,降低引流管脱出断裂风险,有效防治再出血及颅内逆行感染,较单腔引流管引流具有更好疗效。

英文摘要:

Objective To observe the effect of double cavity drainage tubes in treatment of hypertensive cerebral hem- orrhage so as to offer theoratical basisfor the development of a dedicated and effective new drainage device for intracra- nial hematoma treatment in a minimally invasive manner. Methods A total of 104 cases of hypertensive cerebral hem- orrhage patients were randomly divided into two groups from June 2011 to December 2015. The control group consisting of 41 cases used single chamber ventricular drainage pipe; 63 cases formed the experiment group adopting double cavi- ty drainage tube, and the two groups underwent the same operation. The extubation time, hematoma effects, complica- tion and prognosis of two drainage groups served as targets for post-operational were observed. Results The treatment group for hematoma was significantly higher than the control group(P〈0.01), and the catheter removal rate within 5 days of operation was also higher than the control group (P〈0.05), while the control group showed much higher risks of drainage tube clogging, breakage and dropping out than the other(P〈O.05); The experiment group enjoyed greater priority (ADL I grade+ADL I1 grade) than the control group; no statistically significant differences in mortality occurred be- tween the experiment group and the control group(P〉0.05). Conclusion The self-made double cavity drainage tube for minimally invasive treatment of hypertensive cerebral hemorrhage is capable of direct drainage of intracranial hematoma with enhanced drainage effects, simple and convenient drug injection procedure, thus reducing fracture risk of drainage tube dropping out, preventing repeated bleeding and intracranial infection, which leads to better prognosis than single cavity drainage tube drainage.
