目的:研究颞叶癫痫发作期患者过度放电起始区及脑电的传递方向。方法收集8例颞叶癫痫发作(观察组)患者及8例查体健康者(对照组)16导头皮脑电图( EEG)数据,应用Granger因果分析方法,计算频域定向传递函数( DTF)矩阵,判断因果关系;基于DTF计算因果流,确定起始区的位置,观察脑电信号从起始区向其他脑区的传递方向。结果观察组两个脑区的信号传递存在因果关系,过度放电起始区颞区的DTF值为0.06545±00.152,对照组左、右侧颞区的DTF值分别为0.042863±0.0064、0.038488±0.0143,观察组与对照组比较,P均<0.001。因果流值>0.5的区域(有效传出区):2例位于左前中颞区F7、T3;2例位于左中后颞区T3、T5;1例位于左后颞区T5;1例位于左前中后颞区F7、T3、T5;1例位于右中颞区T4;1例位于右中后颞区T4、T6。因果流值<-0.5的区域(有效传入区):左侧6例在前额区Fp1、Fp2或者额区F3、F4有分布;右侧2例不存在因果流值<-0.5的区域。对照组不存在因果流值>0.5或者<-0.5区域。结论颞叶癫痫发作期患者过度放电起始区位于颞区,脑电传递方向从颞区传递到其他脑区,主要集中传递到额区。
Objective For the initiation, and the directional transfer of excessive discharge from epileptic foci to other brain regions, directional transfer pattern of temporal lobe epilepsy EEG was studied .Methods 16 channel EEG data,which was recorded from eight cases of temporal lobe epilepsy group and eight cases of the control group , was calculated the directional transfer function matrix in the frequency domain by Granger causality analysis method .Then,the DTF matrix was verified the au-thenticity of causality, and was excluded pseudo-causality.Causal flow is obtained by calculating the DTF matrix, in order to de-termine the initiation, as well as research EEG directional transfer pattern from initiation to other brain regions .Results there is significant difference between the DTF value (0.06545 ±00.152 ) of the temporal lobe of initiation in epilepsy group and the DTF value (0.042863 ±0.0064) of the left temporal lobe o,r the DTF value (0.042863 ±0.0064) of the right temporal lobe in the normal control group.The causal flow results of the temporal lobe epilepsy group are as follows:the regions of causal flow val-ue〉0.5:the regions of 2 cases'are located on channels F7 and T3 in left tempora, 2 cases'are located on channel T3 and T5 in the left posterior temporal,1 case's is located on channel T5 in the left anterior temporal,1 case's are located on channel F7,T3 and T5 in the left anterior temporal.1 case's is located on channel T4 in right middle temporal, 1 case's are located on channel T4 and T6 in right posterior temporal.The region of causal flow value〈-0.5:the regions of 6 cases'are located on the channel Fp1, Fp2 or F3, F4.There is no region of the causal flow values 〈-0.5 in two patients.The normal control group:there is no significant causal connection between each channel.Conclusions The initiation is located in the temporal lobe.Excessive dis-charge transferred from the the temporal region to other brain regions , mainly in the prefrontal, frontal.
