目的观察培垛普利对兔肥厚心肌室性心律失常的保护作用。方法家兔24只随机分为假手术组,心肌肥厚组和培垛普利干预组。心肌肥厚组和培垛普利干预组通过部分结扎腹主动脉的方法造成兔压力负荷性心肌肥厚模型,假手术组只暴露腹主动脉而不行缩窄术。术后注射青霉素3d,培垛普利干预组给予口服培垛普利,心肌肥厚组与假手术组给予普通饮食。喂养10周后,采用胸骨正中切开暴露心脏,使用程序电刺激方法在体诱导室性心律失常的产生,在体测定室颤阈值。术后测量全心重量、左室重、全心重/体重比值。结果①假手术组未诱发出室性心动过速。心肌肥厚组和假手术组比较,心肌肥厚组室性心律失常的发生率显著升高,而培垛普利干预组室性心律失常的发生率显著降低。②心肌肥厚组室颤阈值较假手术组显著降低,培垛普利干预组室颤阈值较心肌肥厚组显著升高,且培垛普利干预组室颤阈值接近假手术组。③与假手术组相比,心肌肥厚组的全心重、左室重、全心重/体重比值显著增加,培垛普利干预组的上述指标均显著小于心肌肥厚组。结论培垛普利能防止心肌肥大与血管重构,提高室颤阈值,因而有预防恶性室性快速心律失常的作用。
Objective To evaluate the effect of perindopril on ventrieular tachycardia in rabbits with cardiac hypertrophy. Methods A total of 24 rabbits were divided equally into hypertrophy group (LVH was induced by partial ligation of abdominal aorta), sham group (the abdominal aorta was exposed without constriction), and perindopril-treated group (perindopril was administrated after the ligation). Ten weeks later, the heart was exposed in vitro, the development of programmed electrical stimulation (PES) induced ventricular tachycardia and arrhythmic threshold was assessed. Results (1) There was no arrhythmogenesis in the sham group. Compared with sham group, ventricular tachycardia was significantly increased in hypertrophy group, while significantly reduced in perindopril-treated group; (2) Compared with sham group, arrhythmic threshold was significantly reduced in hypertrophy group and arrhythmic threshold was significantly increased in perindopril-treated group; (3) Compared with sham group, the heart weight and heart weight/body weight ratio were significantly increased in hypertrophy group, but significantly reduced in perindopril-treated group. Conclusion Perindopril significantly reduces ventricular tachycardia and arrhythmie threshold, and perindopril also prevents ventricular remodeling in hypertrophy rabbit hearts.
