MRI对可切除性食管胃交界部腺癌侵犯胃裸区的诊断价值

目的 探讨磁共振成像(MRI)对食管胃交界部腺癌(AEG)侵犯胃裸区的诊断价值。方法 利用MRI分析2013年11月至2016年10月经手术、病理证实为食管胃交界部腺癌39例,分析食管胃交界部腺癌对胃裸区的侵犯情况,并与手术病理学结果进行对比。结果 手术病理证实食管胃交界部腺癌侵犯胃裸区有31例,其中MRI正确诊断占30例(96.8%);胃裸区未受侵犯8例,MRI诊断正确占6例(75.0%)。MRI对食管胃交界部腺癌是否侵犯胃裸区的诊断符合率、灵敏度、特异度分别为92.3%(36/39)、96.7%(30/31)、75%(6/8),与病理结果具有高度一致性(Kappa=0.753,P〈0.05)。结论 MRI是判断食管胃交界部腺癌有无侵犯胃裸区的较好办法,能为临床治疗提供可靠依据,对评估患者预后具有重要意义。

英文摘要:

Objective To investigate the value of magnetic resonance imaging (MRI) in diagnosing the gastric bare area (GBA) inviolated by adenocarcinoma of esophagogastric junction (AEG). Methods From November 2013 to Octo- ber 2015,39 patients with AEG confirmed by surgery and pathological diagnosis. The characteristics of the GBA invaded by AEG through MRI, and been compared with surgically pathologic results were analyzed. Results 31 cases with GBA involved by AEG were proved pathologically. 30 cases were correctly determined by MRI (96.8%), and 8 AEG patients without AEG involvement were confirmed pathologically. Among them, 6 cases with AEG correctly determined by MRI (75.0%). The agreement rate, sensitivity and specificity of MRI in depicting the GBA involved by AEG were 92.3% (36/39), 96.7 % (30/31), and 75 % (6/8), respectively, which showed high consistency with pathological results (Kap- pa=0. 753, P〈0.05). Conclusion MRI can be a relatively good method for the detection of whether GBA involved by AEG or not. It can provide reliable evidence for clinical treatment decision making and play an important role in prognosis of the AEG patients.
