正清风痛宁治疗膝骨关节炎的临床研究及对细胞因子的影响

目的探讨正清风痛宁治疗膝骨关节炎的临床疗效及对细胞因子白细胞介素1β和肿瘤坏死因子-α的表达的影响。方法将86例膝骨关节炎患者随机分为试验组和对照组，各43例。试验组采用正清风痛宁60mg／片），60mg／次，2次／d治疗，对照组采用盐酸氨基葡萄糖（240mg／片），240rag／次．2次／d治疗。治疗12N后应用西安大略-麦克马斯特大学骨关节炎调查量表（WOMA6），从关节疼痛、僵硬和活动功能困难程度3个方面进行疗效评价、检测血清白细胞介素1β（IL-1β）和肿瘤坏死因子-α（TNF-α）的表达，同时观察2组不良反应发生率。结果治疗后试验组与对照组WOMAC关节疼痛、僵硬和活动功能困难程度积分均较治疗前下降，差异有统计学意义（P〈0.05）；试验组上述指标改善情况均优于对照组，差异亦有统计学意义（P〈0．05）。治疗后两组临床疗效比较差异有统计学意义（P〈0.05）。两组治疗前后血清IL-1β、TNF-α均下降，试验组较对照组下降明显，差异亦有统计学意义（P〈0．05）。两组不良反应发生率差异有统计学意义（P〈0.05）。结论正清风痛宁能够较快有效缓解膝骨关节炎患者症状体征；能抑制血清IL-1β、TNF-α的表达，是治疗膝骨关节炎的有效方法。

英文摘要：

Objective To observe the effect of zhengqing fengtongning treating knee osteoarthritisand cytokine interleukin-1 β, and tumor necrosis factor-α expression. Methods 86 cases with knee osteoarthritiswere weredivided into two groups.The experimental group was treated with the zhengqing fengtongning（60 mg/tablets,60 mg/times,2 times/d） and the control group were treated using the double finn sodium chloride（240 mg/ tablets,240 mg/times,2 times/d）.The treatment lasted for 128 weeks. Western Ontario McMaster Universities Osteoarthritis survey scale （WOMAC） were used to evaluate by three aspects of the degree of difficulty from joint pain, stiffness, and activities function, interleukin 1β（IL-1 β） and tumor necrosis factor alpha （TNF α ） expression of the patient were observed and the untoward effects in the process of treatment were recorded. Results After treatment, the degree of difficulty from joint pain, stiffness, and activities function in experimental group and control group were all improved, the difference was statistically significant （P〈0.05）; The above indexes were both improved in the experimental group was better than that of control group also have statistically significant difference （P〈0.05）. After treatment,the clinical curative effect of two sets comparing statistical significance （P〈0.05）. Serum IL-1β before and TNF α decreased in after treatment, which was more obvious in experimental group than the control group, also hadstatisticaUy significant difference （P〈0.05）. Adverse reaction rate of two groups of difference was statistically significant （P〈0.05）. Conclusion The zhengqing fengtongning can effectively relieve knee osteoarthritis symptoms and signs; it can inhibit the serum IL-1 β, TNFα expression, is effective method for the treatment of knee osteoarthritis.
