目的:以超声生物显微镜(UBM)观察温经通络法干预胶原诱导性关节炎(CIA)大鼠滑膜增生情况。方法:40只SPF级雌性Wistar大鼠,随机分为正常组、模型组、对照组和中药组,每组10只。正常组常规饲养,其余各组复制胶原诱导性关节炎(CIA)大鼠模型后,分别以蒸馏水、雷公藤多苷片和加味芍甘附子汤干预4周,观察大鼠关节炎症评分(AI)和双足排水体积,超声生物显微镜观察治疗后各组大鼠跖趾关节病变情况,并与组织病理学结果进行对比。结果:造模后14-21 d,除正常组外各组AI值和双足排水体积均显著升高,给药后各组肿胀持续减轻,AI值降低。造模后42 d,中药组大鼠右足排水体积显著低于对照组(P〈0.05),同时期各组间AI和双足排水体积比较差异无统计学意义(P〉0.05);UBM平扫正常组可见骨皮质为连续的、致密强回声带,滑膜无显著增厚,厚度低于0.1 mm,其余3组关节间隙内均可见不同程度中等回声信号,其中模型组骨面毛糙,边缘欠整齐最明显,与病理学结果相符;滑膜病理评分与滑膜厚度呈正相关(r=0.637,P=0.002),与AI评分无显著相关性(r=0.192,P=0.344)。结论:UBM可用于检测RA动物模型早期病理改变,对疗效观察和预后评估具有较好的潜在价值。
Objective: To observe synovial hyperplasia of CIA rats intervened by Wenjing Tongluo therapy by applying ultrasound biomicroscopy. Methods: Forty female SPF Wistar rats were randomly divided into 4 groups: normal group,model group,tripterygium glycosides group( control group) and Chinese medicine group of Modified Shaogan Fuzi( SGFZ) Decoction,10 rats in each group. The normal group were given conventional breeding. The other groups were made collagen induced arthritis( CIA) rat models and gavaged with distilled water,tripterygium glycosides and SGFZ decoction respectively in 4 weeks,and then the arthritis index( AI) and double foot volume of all groups were observed. Ultrasound biomicroscopy( UBM) was applied to observe plantar toe joint disease after treatment of each group,and compared with histopathological results. Results: Except normal group,both AI value and double foot volume of each group were significantly elevated 14 ~ 21 d after immunization,and all groups swelling continued to ease after the treatment and AI value reduced. 42 d after immunization,right foot volumes of Chinese medicine group rats were significantly lower than those of the control group( P〈0. 05). AI and double foot volume had no statistical significance comparing each group during the same time( P〉0. 05). UBM scan showed normal group's continuous cortical bone,dense strong echo band,no signifi-cant thickening,synovial membrane thickness less than 0. 1 mm. The rest three groups' joint gaps had different degrees of medium echo signal and the bone surface coarse and edge neat owe in model group were most serious,which was in conformity with pathological results. Synovial pathological grading and synovial membrane thickness were positively correlated( r = 0. 637,P = 0. 637). There was no significant correlation with AI score( r = 0. 192,P = 0. 192). Conclusion:UBM may be used to detect pathological changes of early RA,whicn may provide the potential value of curative effect observatio
