目的:分析新医改背景下组织资源和社会资源对不同等级医院医生职业承诺的影响,提出针对性的激励措施。方法:采用多阶段分层抽样的方法在山东省各级医院抽取2102名医生进行问卷调查,有效问卷1910份,运用多组回归对不同等级医院医生职业承诺影响因素进行分析。结果:职业成长和社会资源是影响各级医院医生职业承诺的共同因素,且影响程度高于其他影响因素;薪酬及制度公平和工作自主只影响到基层医疗机构医生的职业承诺(β=0.14和0.10,P〈0.05);领导赏识只对二级医院医生的职业承诺有影响(β=0.15,P〈0.01);工作关系因素对各级医院医生的职业承诺均无影响。结论:不同等级医院医生职业承诺的影响因素既有相同之处,又存在差异,随着医院级别提高,医生职业承诺影响因素趋向集中,其中职业成长和社会资源是各级医院医生共同关注的因素。
Objective To analyze the influence of organizational and social resources on doctors' occupational commitment in hospitals of different grades, and put forward incentive measures. Methods 2102 cases of doctors were chosen from hospitals of dif- ferent grades in Shandong Province with multi-stage stratified sampling method and surveyed with questionnaire. 1910 cases of valid questionnaires were obtained and analyzed with multiple-regression. Results Professional development and social resources were the common factors to influence doctors' occupational commitment in hospitals of different grades,which had more significant influence than other factors. Factors of fairness of salary and system and work independence only influenced doctors' occupational commitment in grass-roots hospitals. Leaders' recognition only influenced doctors' occupational commitment in hospitals of grade two. And work- ing relationship had no influence on doctors' occupational commitment in hospitals of different grades. Conclusion The factors influ- encing doctors' occupational commitment in hospitals of different grades were partly similar, which tended to concentrated with the increase of hospitals' grades. And factors of professional development and social resources were commonly concerned.
