酶联免疫吸附试验诊断日本血吸虫人群感染效果的meta分析

目的对酶联免疫吸附试验(ELISA)诊断日本血吸虫人群感染的效能进行系统评价和meta分析。方法在中国知网(CNKI)、万方数据(Wanfangdata)、重庆维普(VIP)、PubMed、Web of Science、Scopus和Google Scholar中检索有关ELISA法诊断日本血吸虫人群感染的全部文献,应用R语言进行meta分析,综合评价ELISA法诊断日本血吸虫人群感染的效能。结果共有9篇符合纳入和排除标准的文献纳入meta分析,文献质量较高。纳入的文献中,ELISA法诊断日本血吸虫人群感染灵敏度为57.1%~97.3%,特异度为20.4%~84.3%;采用随机效应模型加权合并的敏感度、特异度、阳性似然比和阴性似然比分别为84.9%、50.4%、1.666和0.367。应用加权最小二乘法、一般最小二乘法和稳健法计算得出的ELISA合并准确度分别为0.650、0.686和0.666,合并OR值分别为3.432、4.784和3.959,3种方法拟合的ELISA诊断试验SROC曲线下面积分别为0.695、0.741和0.715,加权合并DOR值为4.78。结论ELISA对日本血吸虫人群感染的诊断效能较高,可望为我国消除血吸虫病阶段提供一种值得信赖的免疫诊断工具。

英文摘要:

Objective To systematically review and analyze how effective an enzyme-linked immunosorbent assay(ELISA)is at diagnosing schistosomiasis in humans. Methods Literature regarding the immunodiagnosis of schistosomiasis in humans with ELISA was searched in CNKI,Wanfangdata,VIP,PubMed,Web of Science,Scopus and Google Scholar using the terms" Schistosoma," "schistosomiasis," "diagnosis," and ELISA," with no restrictions on the date of publication.The efficiency with which ELISA diagnosed schistosomiasis in humans was assessed by performing a meta-analysis with R. Results A total of 9sources that met the inclusion and exclusion criteria were included in the final meta-analysis,and all of the sources were high quality.According to the sources included in this study,ELISA had a sensitivity of 57.1%to 97.3% and a specificity of 20.4%to 84.3% at diagnosing schistosomiasis in humans.According to a random effects model,the weighted pooled sensitivity was 84.9%,the weighted pooled specificity was 50.4%,the positive likelihood ratio was 1.666,and the negative likelihood ratio was 0.367.According to weighted least squares,ELISA would have a pooled accuracy of 0.650,apooled OR of 3.432,and an area under the SROC curve of 0.695.According to ordinary least squares,ELISA would have a pooled accuracy of 0.686,apooled OR of 4.784,and an area under the SROC curve of 0.741.According to the robust method,ELISA would have a pooled accuracy of 0.666,apooled OR of3.959,and an area under the SROC curve of 0.715.ELISA would have a weighted pooled DOR of 4.78. ConclusionELISA is efficient at diagnosing schistosomiasis in humans and may provide a reliable tool for immunodiagnosis and elimination of schistosomiasis in China.
