目的分析同型半胱氨酸(Hcy)水平与帕金森病(PD)伴发抑郁症状之间的关系。方法除搜集一般资料外,对两组对象均行UPDRSⅡ运动状态评分、H—Y分期评定、HAMD抑郁量表、MoCA认知功能及血浆Hey、叶酸、维生素B12检测。结果①PD组的Hey值及HAMD评分显著高于健康对照组,而叶酸值及MoCA评分显著低于健康对照组,均具有统计学意义(P〈0.05);②75例PD患者,伴发抑郁症状42例,发生率56.0%(42/75),其中轻中度抑郁为76.19%(32/42);③与PD无抑郁症状组比较,PD伴发抑郁症状组存在运动症状重、病情分期重、认知功能评分低、血浆Hey高、叶酸水平低,具有显著统计学意义:④Hey与HAMD评分呈正相关,相关系数0.556(P=0.00);叶酸与HAMD评分呈负相关,相关系数-0.482(P=0.00);Moca评分与HAMD评分呈负相关,相关系数-0.597(P=0.00)。结论PD伴发抑郁的危险因素包括运动症状重、病情分级重及认知评分低,血浆Hcy高,并且血浆Hey水平越高,认知损害越重,抑郁程度越重。
Objective To analyze the correlation between plasma levels of homocysteine( Hcy) and depression in patients with Parkinson's Disease( PD). Methods 75 PD patients were recorded their general conditions,measured unified PD Rating Scale( UPDRS) Ⅲ and Hoehn-Yahr( H-Y) stages. Meanwhile,25 healthy people were set up as control group. The two groups were all received examination of 24 items of Hamilton Depression( HAMD) scale and Montreal Cognitive Assessment( Mo CA) scale,and measured plasma levels of Hcy,folic acid,and vitamin B12. Meanwhile,the relationship between levels of Hcy and depression in patients with PD were analyzed. Results Patients with PD,contrasted to control group,had a higher level of Hcy,lower levels in folic acid,and a higher scores in Mo CA,all with statistic significance. There were,42 cases with depression in those 75 PD patients( the ratio was 56. 0%),and there were mainly mild and moderate degrees,the ratio 76. 19%( 32 /42). Compared with group PD without depression,the group PD with depression,had severer movement symptoms and H-Y stages,lower scores of cognitive scale,higher levels of Hcy,and lower levels of folic acid,all with statistic significance. The levels of Hcy in PD patients with depression were positively correlated with the scores of HAMD( r = 0. 556, P = 0. 00), levels of folic acid were significantly correlated with the HAMD scores( r =-0. 482,P = 0. 00),and Mo CA scores were negatively correlated with the scores of HAMD( r =- 0. 597,P =0. 00). Conclusion The risk factors of patients PD with depression include severer movement symptoms and H-Y stages,lower cognitive scores,higher levels of Hcy. Whatmore,higher levels of Hcy implies severer cognitive dysfunction and more serious depression degree.
