目的:腹腔脂肪型丝氨酸蛋白酶抑制剂(vaspin)是新发现的脂肪因子,对代谢性疾病具有预防作用。该研究旨在探讨血浆vaspin浓度与冠心病及不稳定型心绞痛的关系。方法:经血管造影诊断明确的冠心病患者88例(其中稳定型心绞痛47例,不稳定型心绞痛41例),同期入院的103例无心脏疾病的患者作为对照。测量患者血浆中vaspin的浓度,记录临床一般情况及血脂、血糖、高敏C反应蛋白等指标。并根据冠状动脉的病变支数来评定冠心病严重程度。结果:不稳定型心绞痛的血浆vaspin浓度(0.43±0.38)μg/L较稳定型心绞痛(0.91±0.95)μg/L显著降低(P〈0.01)。在冠心病患者中血管病变支数与血浆vaspin浓度负相关(r=-0.350,P〈0.01)。ROC曲线分析血浆vaspin对冠心病有鉴别价值(AUC=0.684,P〈0.001),对不稳定型心绞痛也有鉴别价值(AUC=0.640,P=0.024)。结论:不稳定型心绞痛患者具有较低的血浆vaspin浓度;低血浆vaspin浓度与冠心病严重程度呈相关。
Objective:Vaspin was recently identified adipokine,playing a protective role in metabolic disease.The present study aimed to investigate the relationship of vaspin plasma level with stable angina pectoris(SAP) and unstable angina pectoris(UAP).Method:A total of 88 patients with angiographically-provedCAD(SAP 47,UAP 41) and 103 control subjects without cardiovascular diseases were enrolled in this study.Circulating vaspin,clinical parameters,lipid profile,and high-sensitivity CRP were assayed.The severity of CAD was also assessed according to the number of vessel diseased.Result:There are significant difference(P0.01) in circulating vaspin level between SAP([0.91±0.95]ng/mL) and UAP([0.43±0.38]ng/mL).An inverse association between the number of diseased vessels and plasma vaspin was observed(r=-0.350,P0.01) in the CAD group.Construction of receiver operating characteristic curves confirmed that vaspin plasma concentration significantly differentiated CAD patients(AUC=0.684,P0.001),as well as UAP(AUC=0.640,P=0.024).Conclusion:Vaspin plasma levels were decreased in patients with UAP;Low vaspin concentrations correlate with CAD severity.Our findings suggested that vaspin could serve as a novel biomarker of CAD.
