目的观察Ex-press青光眼引流器植入术治疗新生血管性青光眼的疗效。方法选择2012年1月-2014年12月于昆明医科大学第一附属医院眼科住院治疗的新生血管性青光患者,共23例（26只眼）,所有患者均给予经睫状体平坦部玻璃体腔注射Lucentis0.1ml（1.0mg）。注药后3-5d,行Ex-press青光眼引流器植入术。观察术前及术后1周、1个月、3个月、6个月,分别记录两组的平均眼压、视力、并发症情况。结果青光眼引流阀植入术后1周、1个月、3个月、6个月的平均眼压较术前明显降低,分别降低29.93mmHg,31mmHg,31.66mmHg,31.62mmHg,差异有统计学意义（P〈0.01）。术后发生浅前房1例,前房成形渗出2例,前房出血2例,未见脉络膜脱离及眼球萎缩。2例术后高眼压,再次行睫状体冷冻术。结论 Ex-press青光眼引流器植入术对于治疗NVG效果好,手术成功率高,且操作简单,术后并发症少,视力维持好,对于有一定视功能的患者是一种较为安全有效的治疗方法 。

英文摘要：

Objective To observe the results of excessive pressure regulation system（Ex-PRESS） to neovascular glaucoma.Methods 26 eyes in 23 cases with neovascular glaucoma in the First AffiliatedHospital of Kunming Medical University from Jan 2012 to Dec 2014 were analyzed retrospectively. Pan-retina photocoagulations were executed firstly,all patients were treated with intravitreal injec tion of Lucentis 0.1ml（1.0mg） pars plana at first. 3 days latter,26 eyes were treated by-Ex-PRESS. Pre and post-operative（1week,1month,3months,6months） intraocular pressures（I0P）,visual acuity and complications were observed.Results the average IOP reduced greatly,the IOP decreased 29.93 mmHg,31 mmHg,31.66 mmHg,31.62 mmHg in 1 week,1 month,3 months and 6 months in post-operation respectively,the difference was statistically significant（P〈0.001）. After operations,anterior chamber hemorrhage in 2 cases;anterior chamber exudations in 2 cases;shallow anterior chamber in 1 case. intraocular hypertension in 2 cases after operation were observed and reoperated by cyclocryotherapy. eyeball atrophy and chroidal detachment in 0cases were observed respectively. Conclusion Ex-PRESS should be a good method for controlling neovascular glaucoma.Highly successful operation rate,simple manipulation,less complications,better vision keeping,more safety,will be a good selection for NVG.
