目的了解综合医院口腔假丝酵母菌感染现状,分析口腔假丝酵母菌感染和带菌的危险因素。对临床真菌感染的防治提供依据。方法收集我院2009年(6~7)月住院部16个科室579例患者的临床资料,对所有患者进行口腔真菌镜检及培养。将培养有真菌生长的标本分离纯化后应用CHROMagar Candida显色培养基和法国梅里埃API20CAUX酵母菌菌种鉴定试剂条鉴定。对符合入选标准的观察对象,在得到本人知情同意后,采用统一制定的临床资料调查表,由专人对其进行详细的临床检查,并详细记录临床体征及病历资料,最后用SPSS11.5统计分析软件对各种潜在危险因素应用多分类单因素或多因素logistic回归分析,检验水准为α=0.05。结果我院口腔真菌感染患者80例,占13.82%,口腔真菌带菌患者148例,占25.56%,二者合计228例.占总样本39.38%。从临床579例患者口腔分泌物培养获得228份有酵母样菌标本。经分离纯化后获得242株菌株,其中有12例患者同时有2种假丝酵母菌存在。有1例患者同时存在3种假丝酵母菌。住院患者口腔假丝酵母菌感染和带菌年龄分布最多的都在60岁以上年龄组。住院患者口腔假丝酵母菌感染最多的科室为血液科,带菌最多的科室为呼吸科。住院天数、术后住院时间、是否使用抗生素、抗生素使用疗程、是否联合使用抗生素、是否使用糖皮质激素、使用糖皮质激素种类、每日使用糖皮质激素总量及糖皮质激素使用疗程对口腔真菌感染发生的影响有统计学意义(P〈0.05)。年龄和有呼吸道病史对口腔真菌带菌发生的影响有统计学意义(P〈0.05)。结论①住院患者口腔假丝酵母菌感染和带菌年龄分布最多的都在60岁以上年龄组。住院患者口腔假丝酵母菌感染最多的科室为血液科,带菌最多的科室为呼吸科。②住院天数、术后住院时?
Objective To understand the oral Candida infection status quo of general hospital, and analyse risk factors of oral Candida infection and colonization, so provide a strong basis at prevention and treatment of clinical fungal infections. Methods Collected 579 cases clinical data of 16 inpatient departments in our hospital from June to July in 2009, who were performed with oral fungal microscopy detection and fungal culture. Using CHROMagar Candida chromogenic medium and French bioMerieux API 20 C AUX yeast strain identification reagent strips to identify strains after separation and purification from bacterial colony. For observation objects who meet the inclusive criteria, the clinical signs and medical records were collected by professional using the unified clinical data questionnaire, after getting informed consent. The data were analyzed with SPSS11.5 statistical analysis software for a variety of potential risk factors analysis by multiple classification of single factor or multi-factor logistic regression analysis, testing standards for α = 0. 05. Results The hospital oral fungal infection in 80 patients, accounting for 13.82 percent, 148 patients with oral fungal colonization, accounting for 25.56 percent, both the total 228 cases, accounting for 39.38% of samples.
