目的构建较为完善且符合我国国情的社区产后家庭访视护理质量评价指标体系。方法建立科研小组,通过查阅相关文献,以三维质量结构模式为理论框架,构建评价指标体系雏形。选取28名相关领域专家,采用德尔菲法进行专家咨询,并评价咨询专家的积极性、权威性、意见集中程度及意见协调程度。采用界值法进行质量评价指标筛选,采用层次分析法确定各指标权重。结果共进行两轮专家咨询,有效回收率分别为100.0%(28/28)和96.4%(27/28),权威系数分别为0.916和0.904。第一轮专家咨询的127个指标中,重要性评分的算数均数为3.18~4.75,满分比为0.04~0.75,标准差为0.44~1.57;第二轮专家咨询的122个指标中,重要性评分算数均数为3.85~4.89,满分比为0.11~0.89,标准差为0.32~0.77。第一轮专家咨询中,变异率数(CV)〈0.2的指标共107项(84.3%);第二轮专家咨询中,CV〈0.2的指标共122项(100.0%);专家意见的肯德尔协调系数分别为0.173和0.186(P〈0.01)。两轮专家咨询中,各级指标间的Cronbach'sα系数均≥0.752。最终构建出指标体系的一级指标为结构质量、过程质量及结果质量,权重分别为0.240 3、0.549 9和0.209 8;二级评价指标15项,三级评价指标103项。结论该指标体系覆盖内容全面,权重设置科学、客观,为我国社区产后家庭访视护理服务的质量评价提供了一定理论依据。
Objective To establish a well - developed nursing quality evaluation index system of postpartum visit that accords with the national condition of China. Methods A research team was buih, and relevant literatures were reviewed. Using " three - dimensional quality structure" mode as the theoretical framework, we established the rudiment of quality evaluation index system. Twenty - eight experts of relevant fields were enrolled, and expert consultation was conducted using Delphi method. The experts were evaluated by positivity, authority, degree of opinion concentration and degree of opinion coordination. Boundary value method was employed to conduct screening of quality evaluation indexes, and analytic hierarchy process was employed to determine the weight of each index. Results Two rounds of expert consuhation were conducted; the effective returning rate were 100. 0% (28/28) and 96.4% (27/28) respectively and the authority coefficients were 0. 916 and 0. 904. in the first expert consultation, there were 127 indexes; the arithmetic mean of importance score was 3. 18 -4. 75; the ratio of full score was 0.04 - 0. 75 ; standard deviation was 0. 44 - 1.57. In the second round of expert eonsuhation, there were 122 indexes, and the arithmetic mean of importance score was 3.85 -4. 89; the ratio of full score was 0. 11 -0. 89; standard deviation was 0. 32 - 0. 77. In first round of expert consultation, the number of indexes with CV 〈 0. 2 was 107 (84. 3% ) .
