目的观察2型糖尿病(T2DM)病人血糖相关参数糖化血红蛋白(HbA1c)、平均血糖波动幅度(MAGE)变化对血清氧化型低密度脂蛋白受体1(LOX1)、血清晚期糖基化终末产物(AGEs)、晚期糖基化终末产物受体(RAGE)水平的影响。方法纳入T2DM病人33例,进行七点法指端毛细血管血糖测定[三餐前即早餐前(空腹)、午餐前、晚餐前,三餐后两小时及睡前21:00点]计算平均血糖波动幅度;采用离子交换高压液相法检测HbA1c;ELISA法检测AGEs、RAGE、LOX1含量;并进行其他一般性指标检测。以HbA1c作为评价长期血糖控制的监测指标,MAGE作为评价血糖波动性的指标,并观察AGEs、RAGE、LOX1与其相关性。结果与健康对照组相比,T2DM病人血清AGEs、RAGE、LOX1水平显著升高(P<0.01);应用一元线性回归分析显示,T2DM病人血清AGEs、RAGE及LOX1水平均与HbA1c、MAGE均具有显著相关性(P<0.01);进一步进行多元线性回归分析显示,AGEs、RAGE、LOX1与HbA1c、MAGE均有线性关系,均可建立回归方程,且HbA1c标准化回归系数均大于MAGE标准化回归系数。结论血糖波动幅度的增加与2型糖尿病病人血清AGEs、RAGE、LOX1水平的升高密切相关。
Objective To investigate the influence of blood glucose fluctuation on serum advanced glycation end products (AGEs),re- ceptor for AGEs (RAGE),oxidized low density lipoprotein receptor 1(LOX 1)levels in patients with type 2 diabetes mellitus (T2DM).Methods Thirty three diagnosed type 2 diabetic patients with stable glucose lowering treatment were enrolled.The capil- lary glucose of preprandial and 120 min post prandial for each meal and bedtime were measured with an ultrasmart glucose meter(Bayer).The mean amplitude of glycemic excursions (MAGE)were calculated from 7 point daily capillary glucose profiles.HbA1cwas measured with ion exchange high performance liquid chromatography.The levels of serum AGEs,RAGE and LOX 1 were ex- amined by ELISA.The general indexes were measured and then the statistical analysis was conducted.Results The serum LOX 1, AGEs,RAGE levels were increased in patients with T2DM compared with the healthy control(P <0.01).The simple correlation analy- sis showed that there was positive significant correlations between serum LOX 1,AGEs,RAGE levels and HbA1c and MAGE(P < 0.01).Multivariate regression analysis showed that the LOX 1,AGEs,RAGE levels all increased linearly with increasing HbA1c andMAGE,and the standardized partial regression coefficient of HbA1c was higer than MAGE.Conclusion Elevated blood glucose fluc- tuation is closely related to elevated levels of serum AGEs,RAGE and LOX 1 in patients with T2DM.
