目的:了解河南省某农村地区老年人群心血管代谢性危险因素的流行特征,探讨其与脑卒中的关系。方法:采用随机整群抽样的方法抽取河南省某农村地区5459名60岁及以上人群进行横断面调查,通过非条件lo-gistic回归模型和比值比( OR)探讨心血管代谢性危险因素与脑卒中的关联强度。结果:河南省农村老年人群脑卒中的患病率为5.84%。中心型肥胖、TG升高、HDL-C降低、血压升高、FPG升高的患病率分别为45.91%、18.96%、33.67%、64.54%和44.42%;与非脑卒中人群相比,上述心血管代谢性危险因素与脑卒中的关联强度[OR(95%CI)]分别为1.298(1.035~1.628)、1.389(1.065~1.812)、1.607(1.279~2.020)、2.770(2.061~3.725)和1.148(0.915~1.440)。随着心血管代谢性危险因素数量的增加,脑卒中的患病率呈逐渐增加的趋势,其OR(95%CI)分别为2.050(1.118~3.761)、3.087(1.717~5.549)、3.018(1.660~5.488)、3.550(1.892~6.663)和7.256(3.693~14.257)。不同心血管代谢性危险因素在脑卒中和非脑卒中人群中的聚集性差异具有统计学意义( P<0.05)。结论:心血管代谢性危险因素在河南农村老年人群中患病率较高,与脑卒中存在较强的关联性。
Aim:To analyze the epidemic characteristics of cardiometabolic risk factors , and explore the relationship between cardiometabolic risk factors and stroke in elderly rural residents of Henan Province .Methods:A cross-sectional survey was conducted in 5 459 subjects aged 60 years and above by cluster random sampling in a rural area of Henan Prov-ince.Unconditional logistic regression models and odds ratio (OR) were used to identify the correlation between cardiometa-bolic risk factors and stroke .Results:The prevalence of stroke was 5.84%.The prevalence of central obesity ,increase of triglyceride,decrease of high-density lipoprotein-cholesterol,hypertension and fasting plasma glucose rise was 45.91%, 18.96%,33.67%,64.54%,and 44.42%,respectively,and the OR(95%CI) was 1.298 (1.035 -1.628),1.389 (1.065-1.812),1.607(1.279-2.020),2.770(2.061-3.725) and 1.148(0.915-1.440),respectively.Further a-nalysis showed that the OR(95%CI) were 2.050(1.118-3.761),3.087(1.717-5.549),3.018(1.660-5.488), 3.550(1.892-6.663) and 7.256(3.693-14.257) with the increase of number of cardiometabolic risk factors ,respec-tively.In addition,there were significant differences in clustering of different cardiometabolic risk factors between stroke and non-stroke groups(P〈0.05).Conclusion:The prevalence of cardiometabolic risk factors is high in rural elderly residents of Henan Province ,and strong correlations are found between cardiometabolic risk factors and stroke .
