目的 探讨应用扩张后颈横动脉颈段皮支皮瓣修复颈项部瘢痕的临床疗效.方法 对10例颈项部瘢痕患者分别行两期手术治疗.一期在瘢痕同侧颈横动脉颈段皮支皮瓣下置入皮肤软组织扩张器（容量为400 ～ 800 ml）;二期切除颈项部瘢痕并充分松解,采用扩张后颈横动脉颈段皮支皮瓣覆盖其创面;供瓣区均可直接拉拢缝合.结果 所有患者的颈项部瘢痕均得到了有效修复,切口均一期愈合,扩张皮瓣未发生血运障碍,皮瓣颜色、质地与邻近皮肤相似,外形较满意.结论 采用扩张后颈横动脉颈段皮支皮瓣修复颈项部瘢痕是较有效的方法,值得临床推广应用.

英文摘要：

Objective To investigate the clinical efficacy of expanded flaps taken from the cervical cutaneous branch of the transverse cervical artery in the repair of the neck and napex scar.Methods Ten patients with neck and napex scar underwent two-stage surgery.In the first stage of the operation,a skin soft tissue expander with a volume from 400 ml to 800 ml was placed under the flaps of the cervical cutaneous branch of the transverse cervical artery on the same side of the scar.In the second stage of the operation,the scar was excised and fully released and the expanded flap of the cervical cutaneous branch of the transverse cervical artery was used to cover the wound.The donor site was then closed directly.Results All neck and napex scars were repaired effectively,and the incisions healed primarily.No blood disorder occurred in the expanded flap.The color and texture of the flap was similar to the surrounding skin.Patients were satisfied with the post-operative appearance.Conclusion The expanded flap taken from the cervical cutaneous branch of the transverse cervical artery was effective in the repair of the neck and napex scars.It should be considered for promotion in clinic.
