目的:了解国内各地区类风湿关节炎(RA)中医证型的分布特征,揭示其分布规律。方法:通过检索近15年的相关文献,按照不同地域归纳总结,了解RA中医证型在不同地域的分布规律。结果:国内证型总体分布,RA实证者以湿热痹阻、寒湿痹阻和痰瘀互结为主,虚证多见肝肾亏虚、肾气虚寒。不同地域证型分布,东北地区主要证型与全国基本一致,肾气虚寒证较其他地区多;华北地区湿热痹阻证比例明显高于全国,较为突出;华东地区以寒湿痹阻证为最多;华中地区除寒湿痹阻、湿热痹阻外,寒热错杂证较多;华南地区湿热痹阻、寒湿痹阻、肝肾亏虚证排名较全国靠前;西南及西北地区寒湿痹阻证占绝对优势;港澳台地区多见脾肾亏虚、肝肾亏虚、寒湿痹阻证。结论:不同地域RA的中医证型分布存在明显的地域差异。
Objective: To understand the domestic regional distribution characteristic of traditional Chinese medicine (TCM) syndromes of rheumatoid arthritis (RA), and reveal the regularities of distribution. Methods: Related literatures of nearly 15 years were retrieved and summarized according to regions, in order to know the distribution regularities of TCM syndromes of RA in different regions. Results: The general distributions of TCM syndromes of RA were as follows: excess syndromes mainly included syndrome of accumulated dampness-heat, syndrome of blockade of cold-dampness, and syndrome of intermingled phlegm and blood stasis, and deficiency syndromes mainly included syndrome of deficiency of liver and kidney and syndrome of deficiency and cold of kidney qi. The distributions of TCM syndromes of RA in different regions were as follows: the main syndromes of RA in northeast China were basically the same with the nationwide, and the syndrome of deficiency and cold of kidney qi in this region was more than the other regions; the proportion of syndrome of accumulated dampness-heat in north China was apparently higher than that of the other regions of the country; the mainly syndrome of eastern China was syndrome of blockade of cold-dampness; the syndrome of intermingled heat and cold was common in central China in addition to syndrome of accumulated dampness-heat and syndrome of blockade of cold-dampness; in south China, the ranking of syndrome of deficiency of spleen and kidney and syndrome of phlegm-dampness blocking collaterals was at the very top compared with the other regions of the country; syndrome of blockade of cold-dampness occupied absolute advantage position in southwest and northwest China; the common syndromes in Hong Kong, Macao and Taiwan regions were deficiency of spleen and kidney, syndrome of deficiency of liver and kidney and syndrome of blockade of cold-dampness.
