舒芬太尼复合依托咪酯预处理对大鼠心肌缺血-再灌注损伤的保护作用

目的探讨舒芬太尼复合依托咪酯预处理对大鼠心肌缺血-再灌注(I-R)损伤的保护作用。方法 42只大鼠随机均分为七组:假手术组(S组),I-R组,依托咪酯20mg/kg预处理组(M组),舒芬太尼中、高剂量组(SFA1组舒芬太尼2μg/kg预处理、SFA2组舒芬太尼10μg/kg预处理),舒芬太尼中、高剂量复合依托咪酯组(SFMB1组、SFMB2组)。除S组外,其余六组均采用结扎冠状动脉左前降支法制备心肌I-R损伤模型。于实验结束时处死大鼠,采用硫代巴比妥酸法测定血清丙二醛(MDA)浓度,黄嘌呤氧化酶法测定血清超氧化物歧化酶(SOD)活性。结果与S组比较,I-R组SOD活性降低、MDA浓度升高(P〈0.05);与I-R组比较,M、SFA1、SFA2、SFMB1及SFMB2组SOD活性升高、MDA浓度降低(P〈0.05);与SFA1组比较,SFA2组SOD活性升高、MDA浓度降低(P〈0.05);与M组比较,SFMB1组MDA浓度降低(P〈0.05);SFA1和SFMB1组间、SFA2和SFMB2组间上述指标差异均无统计学意义(P〉0.05)。结论不同剂量舒芬太尼和依托咪酯分别预处理均可减轻大鼠心肌I-R损伤,但两者复合处理时心肌保护作用并未增加。

Objective To investigate the protective effect of preconditioning with sufentanil combined with etomidate on myocardial ischemia-reperfusion(I-R)injury in rats.Methods Forty-two adult male SD rats were randomly and equally divided into 7groups of S(sham operation),I-R,M(preconditioned with etomidate 20 mg/kg),SFA1(preconditioned with medium dose of sufentanil2μg/kg),SFA2(preconditioned with high dose of sufentanil 10μg/kg),SFMB1(preconditioned with medium dose of sufentanil plus etomidate)and SFMB2(preconditioned with high dose of sufentanil plus etomidate).Myocardial I-R injury model was established by ligation and reperfusion of the left anterior descending coronary artery in all groups except for group S.Serum concentration of malondialdehyde(MDA)and the activity of serum superoxide dismutase(SOD)were detected by thiobarbituric acid reaction and xanthine oxidase reaction,respectively.Results Compared with group S,serum SOD activity was decreased and MDA concentration was increased in group I-R(P〈0.05).Compared with group I-R,SOD activity was increased and MDA concentration was decreased in groups of M,SFA1,SFA2,SFMB1 and SFMB2(P〈0.05).Compared with group SFA1,SOD activity was increased and MDA concentration was decreased in group SFA2(P〈0.05).Compared with group M,MDA concentration was decreased in group SFMB1(P〈0.05).There were no significant differences in SOD activity and MDA concentration between groups of SFA1 and SFMB1,or between groups of SFA2 and SFMB2(P〉0.05).Conclusion Different doses of sufentanil or etomidate preconditioning can attenuate myocardial I-R injury in rats,while there is no enhancement of myocardial protection when combined use of two drugs.
