目的测量正常头颈部旋转后寰枢关节与椎动脉的相关性,探讨旋转后椎动脉寰枢段的形态改变.资料与方法按照头颈部旋转方位将旋转无异常的157例患者分成中立位组54例、左旋转位组53例、右旋转位组50例;所有患者行头颈部CT血管成像检查,并将扫描数据输入工作站进行三维成像与数值测量,比较3组患者椎动脉寰枢段的形态差异.结果椎动脉寰枢段方向中立位左侧以前倾较多,右侧以后倾较多;旋转位左、右侧倾角较中立位增大,差异有统计学意义(P<0.05).左旋转位时左侧第2弯曲高度较右旋转位及中立位增大,右侧第2弯曲高度较右旋转位及中立位减小,差异均有统计学意义(P<0.05);右旋转位时右侧第4弯曲宽度及高度较左旋转位及中立位减小,差异有统计学意义(P<0.05).结论头颈部旋转可导致椎动脉的形态改变,椎动脉倾角大小可以影响椎动脉的伸缩程度,第2、4弯曲的改变可能影响椎动脉血流.
Purpose To measure the correlation of atlantoaxial joint and vertebral artery (VA)with head-neck rotation, and to explore the morphological changes of atlantoaxial segment of VA. Materials and Methods Totally 157 patients without abnormal head- neck rotation were randonlly divided into three groups including neutral (n-54), left rotation (n-53) and right rotation (n-50) and underwent head-neck CT angiography (CTA). These parameters were analyzed on a workstation for three-dimensional imaging and measurements to compare the morphological difference in the atlantoaxial segment of the vertebral artery. Results The inclination angle of VA was greater in left and right rotation compared to that in neutral position (P〈0.05). In lelt rotation, the height of left second curve was greater than the other two groups, while the height of right second curve was less than the other two groups (P〈0.05). In right rotation, the width and height of right fourth curve were less than the other two groups (P〈0.05). Conclusion Head-neck rotation causes morphological changes of VA, the inclination angle of VA can affect its scalability, the change of second and fourth curve may affect blood stream.
