目的:评价牙周膜牵张快速移动尖牙对牙根吸收、牙髓活力、牙周组织的影响。方法:11名前牙拥挤或前突需要减数正畸治疗的病人共20个上颌尖牙,在拔除第一前磨牙的同时进行尖牙远中牙槽间隔减阻手术,术后黏固自制的牵张器。术后3d开始牵张加力,3次/d,0.1mm/次。在矫治开始前、牵张结束时和正畸治疗结束时分别拍摄被牵张尖牙根尖片评估其牙根吸收情况,检测被牵张尖牙牙髓电活力和探诊深度。结果:被牵张的20个尖牙中,有6个在牵张结束时有I度根吸收,在矫治结束时,8个表现为I度根吸收;牙髓电活力测试值在牵张结束时较牵张前有所减小(P〈0.05),矫治结束时与牵张前接近(P〉0.05);快速牵张结束时,被牵张尖牙远中位点探诊深度增加,在矫治结束时又有所降低。结论:牙周膜牵张未造成被牵张尖牙根尖、牙髓、牙周的不可逆损伤,是一种安全有效的快速尖牙移动方式。
AIM : To evaluate the effects of rapid canine distalization on canine pulp vitality, periodontium and root resorption. METHODS: Twenty canines in 11 patients who needed first premolar extractions were involved. A tooth-borne, custom-made distractor was bonded right after the premolar was extracted and the interseptal bone resistance reduced. Three clays post-operation, the distractor was activated 0.1 mm, 3 times a day. Orthodontic models, panoramic radiographs, periapical radiographs, electrical vitality test and probing depth (PD) were assessed before and after the distraction procedure and after the completion of the orthodontic treatment. RESULTS : Six out of 20 canines showed root resorption of I degree at the end of distraction, and 8 out of 20 canines showed root resorption of I degree after the orthodontic treatment. The pulp vitality of canines was lowered at the end of distraction ( P 〈 0.05 ) , while it returned to normal at the end of orthodontic treatment( P 〉 0.05 ). The PDs of distal points of distracted canine were increased after distraction, which were decreased afterwards ( P 〈 0.05 ). CONCLUSION : The rapid canine distalization through distraction of the the periodontal ligament after the reduction of interseptal bone resistance is a safe and effective approach to move canines rapidly.
