目的 观察 ω-3多不饱和脂肪酸对胃癌手术患者热休克蛋白70(HSP70)、炎性反应及免疫状态的影响.方法 接受胃癌切除手术患者120例进入研究,实验组60例接受ω-3多不饱和脂肪酸补充的全胃肠外营养,对照组60例接受一般全胃肠外营养.7d后观察HSP70、肿瘤坏死因子-α(TNF-α)及CD4/CD8变化.结果 两组患者研究期间均未出现死亡及过敏等不良反应.外周血中CD4/CD8比值在实验组手术当日是1.57±0.42,用ω-3鱼油脂肪乳7d后是3.97±0.31,明显升高,差异有统计学意义(P<0.05).外周血中TNF-α在实验组手术当日是(334.83 ±61.37) pg/mg;用ω-3鱼油脂肪乳7d后是(176.46±51.61) pg/mg,明显降低,差异有统计学意义(P<0.05).外周血中HSP70在实验组手术当日是(4.19 ±0.56) g/L;用ω-3鱼油脂肪乳7d后是(19.13 ±3.17) g/L,明显升高,差异有统计学意义(P<0.05).而对照组外周血中CD4/CD8比值、TNF-α及HSP70表达在手术前后无明显变化,差异无统计学意义(P>0.05).结论 胃癌手术患者术后使用ω-3多不饱和脂肪酸可增加HSP70表达,减轻患者炎性反应,并改善患者免疫功能状态.
Objective To evaluate the effect of ω-3 polyunsaturated fatty acid on heat shock protein 70 (HSP 70) and tumor necrosis factor-α (TNF-α) in gastric cancer patients after operation.Methods 120 gastric cancer patients after operation were randomly divided into two groups: the group of standard total parentaral nutrition (standard group, n =60) and the group of standard total parenteral nutrition with ω-3 polyunsaturated fatty acid (treatment group, n =60).HSP70, TNF-α and CD4/CD8 ratio were measured.Results HSP70 and CD4/CD8 ratio were significantly hingher in treatment group after ω-3 polyunsaturated fatty acid nutrition treatment for 7 days (P 〈 0.05).TNF-α was significantly lower in treatment group after 7-day ω-3 polyunsaturated fatty acid nutrition treatment (P 〈 0.05).However, there was no significant change in standard group (P 〉 0.05).Conclusion ω-3 polyunsaturated fatty acid administration is well tolerated in surgical gastric cancer patients and is beneficial.These results indicate that ω-3 polyunsaturated fatty acid can directly attenuate pro-inflammatory cytokine release, and improve immune responsiveness.
