目的探讨人工全膝关节置换术患者手术前后应用不同角度股四头肌板进行负重训练对患肢股四头肌肌力和关节功能康复的影响。方法选择2010年9月-2011年2月本科室收治的36例行单侧人工全膝关节置换术患者,随机分为实验组和对照组,每组各18例。对照组采用常规股四头肌等长收缩训练,实验组应用股四头肌板和砂袋进行股四头肌负重训练,比较两组患者术前1d和术后第14d膝关节活动范围(Range Of Motion,ROM)和美国膝关节学会评分(American Knee Society,AKS)的差异,以及两组患者术后达到直腿抬高的时间。结果实验组患者术后第14d ROM大于对照组,AKS评分高于对照组,达到直腿抬高的时间短于对照组(均P〈0.05)。结论人工全膝关节置换术患者手术前后应用可调不同角度的股四头肌板进行负重训练,可加快患肢股四头肌肌力和关节功能康复。提高治疗效果。
Objective To study the effect of weigh-loading exercise of quadriceps femoris on rehabilitation of myodynamia and joint function after total knee arthroplasty (TKA). Methods 36 cases of unilateral TKA patients were evenly randomized into intervention group and control group. The patients in the intervention group took weigh-loading exercise on quadriceps femoris and those controls took the isometric contraction exercise on quadriceps femoris. The two groups were compared before and after surgery in respect of joint range of motion, scores by American knee society and the time for straight leg raising when they could raise their legs 1 day before TKA and 14 days after that. Results The AKSS and ROM significantly increased compared to the control group, while the time for straight leg raising in the intervention group was significantly shorter than that of the control group (all P 〈 0.05). Conclusion Pre- and post-TKA weigh-loading exercise of quadrieeps femoris can speed up the rehabilitation of myodynamia and joint function.
