目的 探讨N端脑钠肽前体(N-terminal pro-brain natriuretic peptide,NT-proBNP)检测在小儿川崎病随访中的意义.方法检测146例川崎病(Kawasaki disease,KD)患儿急性期、病程2~4周、2个月、6个月、1年、2年血浆NT-proBNP,同时行心脏彩超检查,根据冠状动脉有无病变情况分为冠状动脉病变组和无冠状动脉病变组,并对2组血浆NT-proBNP进行比较.结果 146例KD患儿中无冠状动脉病变组88例,血浆NT-proBNP均在2周内恢复正常;有冠状动脉病变组58例,血浆NT-proBNP恢复时间较无冠状动脉病变组慢;冠状动脉病变组急性期血浆NT-proBNP显著高于无冠状动脉病变组;有冠状动脉病变组和无冠状动脉病变组急性期血浆NT-proBNP行ROC曲线分析显示血浆NT-proBNP曲线下面积0.823(95% CI为0.754~0.893).结论 KD患儿血浆NT-proBNP可作为预测冠状动脉病变的指标,在KD的随访过程中有重要意义,特别是当NT-proBNP〉822.35ng/L时更应注意是否有冠状动脉损害.
Objective To observe the change of N-terminal pro-brain natriuretic peptide(NT-proBNP) in children with Kawasaki disease (KD) and then further discuss the significance of NT-proBNP in the follow-up of KD.Methods The plasma NT-proBNP of 146 cases of children with KD was tested separately in their acute stage,2-4 weeks,2 months,6 months,1 year,2 years,while at the same time ultrasound cardiogram examination was made in the children.Then the children were divided into coronary artery lesion group and non coronary artery lesion group according to with or without coronary artery disease,and the plasma NT-proBNP in two groups was compared.Results Eighty-eight cases of non coronary artery lesion group in 146 KD cases recovered to normal plasma NT-proBNP in two weeks,while the other 58 cases with coronary artery lesion in 146 KD children,the plasma NT-proBNP recovered more slowly than those in non coronary lesion group.And in the acute-stage,plasma NT-proBNP of coronary lesion group was significantly higher than that of non coronary lesion group.The analysis of ROC curve of the acute phase plasma NT-proBNP of coronary lesion group and non coronary lesion group showed 0.823 (95% CI 0.754-0.893) of the plasma NT-proBNP curve area.Conclusion The plasma NT-proBNP could be used as a predictive index of coronary artery lesion with KD,and the plasma NT-proBNP was important in KD follow-up process.Especially when the index of NT- proBNP is more than 822.35ng/L with KD,the patients should be more concerned to see whether or not coronary artery damage.
