目的探讨平衡针对腰椎间盘突出根性神经痛大鼠的抗炎、镇痛免疫调节作用及机制。方法模拟腰椎间盘突出症神经根压迫症状,取临床常用的腰痛穴、臀痛穴做平衡针治疗的刺激点,选用行为学、组织学及放射免疫法等方法,观察平衡针治疗的腰椎间盘突出根性神经痛症大鼠耐痛阈、神经根组织病理变化及下丘脑β-内啡肽(β-EP)、血清白细胞介素-1β(IL-1β)含量的变化。结果腰椎间盘突出神经根压迫症状大鼠模型患侧的耐痛阈降低,经平衡针治疗后耐痛阈明显升高。平衡针可明显改善腰椎间盘突出根性神经痛引起的神经根水肿和脱髓鞘变化,缓解根性神经痛症状。模型组大鼠IL-1β、β-EP含量明显升高,平衡针治疗后含量显著下降。结论提示平衡针对腰椎间盘突出症状有抗炎、镇痛效应。由于平衡针对刺激点的针刺作用,通过改变神经系统的兴奋性,启动了神经-内分泌-免疫系统,进而引起全身性的抗炎和镇痛作用,使机体建立了新的神经、内分泌、免疫的平衡状态,进而调整机体的各项生理功能,减轻神经根的水肿以及局部炎症,最终促进病损神经根损伤的修复。
