目的了解医院志贺菌属的菌群分布及耐药情况,以指导临床合理用药。方法采用国际标准平皿琼脂对倍稀释法对2009年9月安徽医科大学第一附属医院临床标本分离的30株志贺菌耐药性进行分析,并采用生化和血清学方法鉴定血清群。按照2009年美国临床实验室标准委员会(CLSI)指导原则的标准判定细菌对抗生素的敏感率(S%)、中介率(I%)和耐药率(R%)。结果 30株志贺菌属中有福氏志贺菌23株,宋内氏志贺菌6株,鲍氏志贺菌1株。碳青霉烯类抗生素是对志贺菌属抗菌作用最强的一类药物。头孢吡肟、头孢他啶、哌拉西林/三唑巴坦、阿米卡星对其有很好的抗菌活性。志贺菌属对头孢哌酮、头孢噻肟、头孢西丁、哌拉西林、庆大霉素及喹喏酮类药物仍有较高的耐药率。结论从该院临床分离的志贺菌属对喹诺酮类和第三代头孢类抗生素有一定耐药性,应加强监控。
Objective To investigate the distribution and the resistant rate of Shigella Within the First Affiliated Hospital of Anhui Medical University in order to guide the choosing for antimicrobial agents.Methods Thirty strains of Shigella were isolated from the Clinical samples in Sept 2009.The groups were identified by biochemical and serologic tests.Susceptibility of these 30 strains of Shigella to various antibiotics was tested by the standardized custom dilution MIC panels.The susceptible rate(S%),intermediate rate(I%) and resistant rate(R%) of drugs were evaluated according to the criteria in guidelines of CLSI(2009).Results There were 23 strains of Shigella flexneri,6 strains of S.sonnei and 1 strains of S.boydii among 30 strains of Shigella.Carbapenems were the most active antimicrobial agents tested against Shigella.Cefepime,ceftazidime,piperacillin / tazobactam and amikacin showed excellent activity against Shigella.Shigella was still resistant to cefoperazone,cefotaxime,cefoxitin,piperacillin,gentamicin and fluoroquinolones.Conclusion There is a certain resistant rate of the Shigella to fluoroquinolones and the third generation cephalosporins.More attention should be paid to the surveillance and control of such resistance.
