目的:探讨激素诱发股骨头坏死脂质代谢变化规律,寻求中药干预作用"靶点"及作用机制。方法:通过激素诱发股骨头坏死实验兔模型,基于超高效液相色谱(UPLC)技术,观察与体内脂质代谢密切相关的游离脂肪酸(FFA)在激素作用下的变化规律;生化分析对血清脂质和脂蛋白的影响;观察股骨头骨组织形态学的相应改变。结果:在3周和5周的时间点上,实验动物血浆中FFA呈先高后低现象,3周时与正常对照组比较,均显著升高(P〈0.01);第5周时中药组、西药组的FFA仍高于模型组(P〈0.05);血浆中的胆固醇(TC)、三酰甘油(TG)呈相应变化;模型组骨组织病理有骨坏死表现。结论:FFA代谢异常是激素诱发股骨头坏死的重要病理基础;中药通过调节脂质代谢可以早期预防激素性股骨头坏死。
Objective: To investigate the lipid metabolism change rule in the progresssion of steroid-induced avasular necrosis of femoral head, and look for the target of Chinese medicine intervention, observe the effect of early intervention. Methods: After setting up rabbit model of steroid-induced avasular necrosis of femoral head, we observe the change rule of free fatty acids with hormone that closely related lipid metabolism; analyze the influence such as serum lipid, proteolipid protein by laboratory tests; and observe appropriate change under filters and electron microscope of local femoral head. Results: The free fatty acids of experimental animals increased before decreasing in the time of 3 weeks and 5 weeks, compared with normal group, it's increased significantly in 3 weeks (P〈0.01), while the free fatty acids of Chinese medicines group and western group still higher than model group (P〈0.05) in 5 weeks. Cholesterol and triacylglycerol was related positively to the changes of free fatty acids, and pathology examination found osteonecrosis femoral head in model group. Conclusion: Abnormal metabolism of free fatty acid is an important pathological basis of femoral head necrosis. Traditional Chinese medicine can prevent early of femoral head necrosis by regulate lipid metabolism.
