目的：评估急性心肌梗死患者介入围手术期强化应用瑞舒伐他汀对造影剂肾病的防治作用。方法：选取2012-2013年本院收治的急性心肌梗死行急诊PCI手术的患者202例,分为瑞舒伐他汀组100例,对照组102例,瑞舒伐他汀组患者从入选开始每晚服用瑞舒伐他汀20 mg,并持续至术后3 d,后改为10 mg/d直至出院;对照组患者术前服用瑞舒伐他汀10 mg,直至出院。患者于术前、术后72 h测定患者的血流变指数,尿结晶计数及分类,尿蛋白/尿β2微球蛋白比值,同时观察患者造影剂肾病的发生率。结果：瑞舒伐他汀组造影剂肾病的发生率为6.00%,明显低于对照组的11.76%,比较差异有统计学意义（P〈0.05）。血流变指数、尿结晶计数及尿蛋白/尿β2微球蛋白比值与造影剂肾病高度相关,瑞舒伐他汀组可显著降低上述指标。结论：在急性心肌梗死介入围手术期强化应用瑞舒伐他汀可显著降低造影剂肾病的发生率,瑞舒伐他汀可通过降低患者尿造影剂结晶计数与造影肾病的发病密切相关。

英文摘要：

Objective： To assess the preventive and therapeutic effect of rosuvastain used in acute myocardial infarction （ AMI ） patients during PCI perioperative period. Method： From 2012 to 2013, 202 patients were divided into rosuvastain group （ n=100 ） and control group （n=102） . The rosuvastain group was treated by intake of rosuvastain 20 mg/d every night for four days （ one day before and three day post operation ）, and 10 mg/d every night till outward. While the control group intake rosuvastain 10 mg/d every night lasted till outward. The urine crystals＇ count ＆ assortment, protein/β2 microprotein ratio, blood theology were measured pre and post operation for the next 72 h. The incidence of contrast-induced nephropathy （ CIN ） was simultaneously observed. Result： Baseline data were similar between the two groups. The rosuvastain group had a significantly lower incidence of CIN compared with that control group. Urine crystals＇ count and classification were highly related with the CIN, as well as the rosuvastain group remarkably reduced the above index. Also, the urine protein/β2 microprotein ratio, blood rheology were disclosed the same results. Conclusion： Rosuvastain could significantly reduce the CIN in the AMI-PCI patients by decreasing the urine crystal, protein/β2 microprotein ratio and improving the blood rheology.
