目的针对《全国地方病防治规划(2011-2015年)》中克山病防控的目标为基本消除克山病,探索克山病消除评估的研究方法。方法应用全人群克山病病例搜索和重点人群调查相结合方法。全人群病例搜索是在元宝镇全部村村医上报克山病疑似病例与受威胁人口资料,对疑似病例进行诊断核实,重点人群调查是在克山病病情最重的村做重点调查,获得各型克山病病例,同时进行危险因素调查,采集重点调查村中成人和儿童枕部距发根2 cm发样2 g,测量发硒水平。对尚志市疾病预防控制中心进行克山病防控措施及其落实情况问卷调查。结果元宝镇共有人口16 487人,常住人口14 862人,未检出急型、亚急型克山病病例,检出慢型克山病10例,患病率6.7/万,其中男性6例,患病率为6.5/万,女性5例,患病率为6.9/万;潜在型克山病调查点共调查常住人口399人,其中男性150人,女性249人,共检出潜在型克山病10例,患病率为2.5%,其中男性4例,患病率为2.7%,女性6例,患病率为2.4%,均高于全国非克山病病区扩心病调查中类潜在型克山病0.9%的本底值。发硒水平为(0.230±0.179)mg/kg,其中12例发硒水平〈0.12 mg/kg,占23.5%,4例发硒水平〈0.05 mg/kg,占7.8%。结论该研究方法可满足克山病消除评估的需要,通过此评估方法,元宝镇克山病病情仍未达到消除水平。
Objective To achieve the Keshan disease( KD) prevention and control target that Keshan disease is eliminated basically and the endemic villages are eliminated to more than 90% in the National Endemic Disease Control Plan( 2011- 2015),exploring the assessment methods. Methods The KD condition in Yuanbao town was got by the whole population searching and the key population investigation. The acute, subacute and chronic KD cases were got by the whole population searching. All the suspected cases were collected by reported by village doctor and case searching, but confirmed by investigation team. The latent KD condition was got by the key population investigation. According to the previous KD disease monitoring and the chronic Keshan disease survey results, the village which the KD condition was the most serious was selected as the key spot and its inhabitants were the respondents. The threatened population data was collected by village doctor by the questionnaire. The hair selenium was test by the hydride atomic fluorescence spectrometry. The KD prevention and control measures and implementation of the Shangzhi Center for Disease Control and Prevention( CDC) was collected from the head of Shangzhi CDC by the questionnaire.Results Yuanbao town has a population of 16,487 people, and the resident population of 14,862 people. There is no acute and subacute KD disease case, but 10 chronic KD cases. The chronic KD prevalence is 6. 7 /10,000, including 6 males( 6. 5 /10,000) and 5 females( 6. 9 /10,000). A total of 150 men and 249 women were screened in the key investigated village. The latent KD prevalence is 2. 5%, including 4 males( 2. 7%) and 6 females( 2. 4%). It's higher than 0. 9% which is from the national survey of the dilated cardiomyopathy. The hair selenium level is( 0. 230 ± 0. 179) mg / kg, including 12 cases are less than 0. 12 mg / kg, accounted for 23. 5%, and 4 cases are less than 0. 05 mg / kg, accounting for 7. 8%.
