目的探讨腹腔镜与开腹根治术对早期宫颈癌患者术后生存状况的影响。方法收集2007年7月至2009年12月于新疆医科大学附属肿瘤医院行宫颈癌根治术的ⅠIB1-ⅡA2期宫颈癌患者115例。随访5年，失访22例，失访率19．1％，有效病例93例。根据宫颈癌根治术方式的不同，将患者分为腹腔镜组（60例）和开腹组（33例）。记录并比较2组患者术后3、5年无瘤生存率、复发率、转移率及生存率。结果腹腔镜组3年无瘤生存率、复发率、转移率和生存率与开腹组比较[81．7％（49／60）比81．8％（27／33）、10．0％（6／60）比9．1％（3／33）、8．3％（5／60）比9．1％（3／33）、93．3％（56／60）比93．9％（31／33）]，差异均无统计学意义（均P〉0．05）。腹腔镜组5年无瘤生存率、复发率、转移率和生存率与开腹组比较[75．0％（45／60）比72．7％（24／33）、13．3％（8／60）比15．2％（5／33）、11．7％（7／60）比12．1％（4／33）、83．3％（50／60）比84．8％（28／33）]，差异均无统计学意义（均P〉0．05）。结论腹腔镜根治术与开腹根治术比较，具有相似的无瘤生存率、复发率、转移率及生存率。

英文摘要：

Objective To investigate effects of laparoscopic and open radical surgery on .postoperative survival in patients with early cervical cancer. Methods Totally 115 patients who underwent radical surgery for Ⅰ B1-Ⅱ A2 cervical cancer from July 2007 to December 2009 in Tumor Hospital Affiliated to Xinjiang Medical University were enrolled; 93 patients were successfully followed up for 5 years, the loss rate was 19. 1% （22/115） ; 60 patients had laparoscopic surgery and 33 patients had open surgery. The 3-, 5-year disease-free survival, recurrence, metastasis and survival rates were analyzed. Results There were no significant differences of 3-year disease-free survival, recurrence, metastasis and survival rates between laparoscopic surgery group and open surgery group[81.7% （49/60） vs 81.8% （27/33）, 10.0% （6/60） vs 9. 1% （3/33）, 8.3% （5/60） vs 9. 1% （3/33） , 93.3% （ 56/60 ） vs 93.9% （ 31/33 ） ] （ P 〉 0. 05 ）.
