目的探讨诺氟沙星治疗大肠埃希菌感染性腹泻的临床疗效,提高大肠埃希菌感染的治疗效率。方法选取2011年3月-2013年3月收治的感染性腹泻患者共232例,将入选患者分为观察组115例和对照组117例;观察组患者采用诺氟沙星联合双歧四联活菌治疗,对照组单纯采用诺氟沙星治疗。结果 232株大肠埃希菌中致病性大肠埃希菌(EPEC)135株占58.2%,侵袭性大肠埃希菌(EIEC)73株占31.5%,产毒性大肠埃希菌(ETEC)24株占10.3%;其中EPEC型和EIEC型对诺氟沙星、氯霉素的耐药率较低,分别为28.15%和30.37%、21.92%和26.03%;ETEC对头孢吡肟、头孢曲松以及诺氟沙星的耐药率较低,分别为20.83%、33.33%及37.50%;观察组患者腹泻停止时间和体温恢复正常时间明显要短于对照组患者;观察组患者的白细胞总数(WBC)5-10/HP的病例数明显要少于对照组患者,WBC〈5/HP的病例数则多于对照组患者,两组比较差异有统计学意义(P〈0.05);观察组的显效率为63.48%、治疗总有效率为92.17%,明显高于对照组的42.74%、88.03%,两组比较差异有统计学意义(P〈0.05)。结论诺氟沙星可有效治疗大肠埃希菌感染性腹泻,致泻性大肠埃希菌对诺氟沙星的耐药率低。
OBJECTIVE To explore the clinical effect of norfloxacin on treatment of infectious diarrhea due to Escherichia coli so as to raise the effective rate of treatment of E.coli infection.METHODS A total of 232 patients with infectious diarrhea who were treated in the hospital from Mar 2011 to Mar 2013 were enrolled in the study and divided into the observation group with 115 cases and the control group with 117 cases.The observation group was treated with norfloxacin combined with bifidobacterium tetravaccine,while the control group was treated with single norfloxacin.RESULTS Of 232 strains of E.coli,135(58.2%)were enteropathogenic E.coli(EPEC),73(31.5%)were enteroinvasive E.coli(EIEC),and 24(10.3%)strains were enterotoxigenic E.coli(ETEC).The drug resistance rates of the EPEC to norfloxacin and chloromycetin were 28.15% and 30.37%,respectively;the drug resistance rates of the EIEC were 21.92% and 26.03%,respectively.The drug resistance rates of the ETEC to cefepime,ceftriaxone,and norfloxacin were 20.83%,33.33%,and 37.50%,respectively.The stopping time of diarrhea and the recovery time of body temperature of the observation group were significantly shorter than those of the control group.The number of cases with the total white blood cells(WBC)counts ranging between 5and 10/HP was significantly less in the observation group than in the control group;the number of cases with the total WBC counts less than 5/HP was significantly larger in the observation group than in the control group(P〈0.05).The effective rates of the observation group was 63.48%,significantly higher than 42.74% of the control group;the total effective rate of treatment of the observation group was 92.17%,significantly higher than88.03% of the control group(P〈0.05).CONCLUSION Norfloxacin can achieve significant effect on treatment of the infectious diarrhea due to E.coli.The drug resistance rate of the diarrheagenic E.coli is low.
