目的测定肠易激综合征(irritable bowel syndrome,IBS)患者血清及结肠黏膜中肿瘤坏死因子-α(TNF-α)、白介素-10(IL-10)和紧密连接蛋白-1(claudin-1)的表达水平及其相关性,进一步了解IBS的发病机制。方法对22例IBS腹泻型(D-IBS)、18例IBS便秘型患者(C-IBS)、20例健康对照者应用ELISA法检测血清中TNF-α、IL-10、claudin-1的含量,免疫组化法检测结肠黏膜TNF-α、IL-10、claudin-1的表达。结果 1血清中TNF-α水平及结肠黏膜表达率D-IBS组均高于正常对照组(P〈0.05);C-IBS组与正常对照组相比差异无统计学意义(P〉0.05)。2血清IL-10水平及结肠黏膜表达率D-IBS组均低于正常对照组(P〈0.05);C-IBS组与正常对照组相比差异无统计学意义(P〉0.05)。3血清claudin-1水平及结肠黏膜表达率D-IBS组均低于正常对照组(P〈0.05);C-IBS组均高于正常对照组(P〈0.05)。4D-IBS组血清TNF-α与claudin-1成负相关(r=-0.867,P〈0.05),血清IL-10与claudin-1成正相关(r=0.868,P〈0.05)。结论 IBS腹泻症状的产生可能与炎性因子失衡而导致claudin-1改变有关。
Objective To explore the pathogenesis of irritable bowel syndrome(IBS) by detecting the levels of tumor necrosis factor-α (TNF-α), interleukin-10(IL-10) and tight junction protein-1 (claudin-1) in serum and colonic mucosa of patients with IBS and analy- zing their correlation. Methods The serum levels of TNF-α, IL-10 and claudin-1 were detected in 22 cases of diarrhea-type IBS (D- IBS), 18 cases of constipated-type IBS(C-IBS), and 20 healthy controls by ELISA. Meanwhile, the expression of TNF-α, IL-IO and claudin-1 was detected in colonic mucosa by immunohistochemistry. Results ①The level of TNF-ct was significantly higher in the se- rum and colonic mucosa in D-IBS group than that in control group(P 〈 0.05 ), but there was no significant difference in the serum and the colonic mucosa between C-IBS group and control group ( P 〉 0.05 ). ②The level of IL-10 in D-IBS group was significantly lower than that in control group ( P 〈 0.05 ), but there was no significant difference in the serum and the colonic mucosa between C-IBS group and control group(P 〉 0.05 ). ③The level of claudin-1 in D-IBS group was significantly lower in the serum and the colonic mu- cosa than that in control group( P 〈 0.05 ), but it was significantly higher in C-IBS group than that in control group( P 〈 0.05 ). ④The TNF-α level was negatively correlated with claudin-1 level in D-IBS group( r = - 0. 867 ,P 〈 0.05 ) ,while the IL-10 level was positively correlated with claudin-1 level( r = 0. 868,P 〈 0.05 ). Conclusion The changes of claudin-1 are caused by the imbalance of inflam- matory cytokines ,which may be related to the symptom of diarrhea.
