目的总结腕关节镜技术在Ⅰ型TFCC损伤中的治疗效果。方法对14例Ⅰ型TFCC损伤的患者(Palmer分型ⅠA型8例,ⅠB型4例,ⅠD型2例)分别进行关节镜手术治疗,ⅠA型损伤进行清创术处理,ⅠB型损伤及ⅠD型损伤均进行创面新鲜化及镜下缝合术处理,对术前及术后最后一次随访进行Cooney评分并统计分析。结果术后随访5~24个月,参照Cooney腕关节功能评分标准,术前疼痛评分14.29±3.85,功能评分17.50±6.12,活动度评分19.29±5.14,握力评分21.43±4.97,Cooney总评分72.50±8.72。末次随访时疼痛评分23.57±3.63,功能评分22.14±4.69,活动度评分22.86±4.26,握力评分24.29±2.67,Cooney总评分92.86±10.69,与术前评分相比,均具有显著统计学意义。临床效果评定优8例,良4例,可2例,优良率达到85.71%。结论在PalmerⅠ型TFCC损伤的治疗中,腕关节镜技术安全且疗效确切。
Objective To summarize the treatment effects of wrist arthroscopic treatment in Palmer typeⅠtriangular fibrocartilage complex(TFCC)injuries.Methods Fourteen patients(8 of Palmer typeⅠA,4 of Palmer typeⅠB,2 of Palmer typeⅠD)of TFCC injuries were diagnosed and treated with wrist arthroscopic technique.Different injuries were treated with different techniques.Patients of Palmer typeⅠA were treated with debridement.Patients of Palmer typeⅠB and Palmer typeⅠD were treated with suturing skills under wrist arthroscope.Then the postoperative and preoperative wrist joint Cooney ratings were analyzed.Results Fourteen patients were followed between5-24 monthes.The preoperative Cooney scores were 14.29 ± 3.85 at wrist pain,17.50 ± 6.12 at wrist function,19.29 ± 5.14 at wrist activity,21.43 ± 4.97 at gripping power,and 72.50 ± 8.72 at total score.The Cooney scores at last follow-up were 23.57 ± 3.63 at wrist pain,22.14± 4.69 at wrist function,22.86± 4.26 at wrist activity,24.29± 2.67 at gripping power,and 92.86 ± 10.69 at total score,showing remarkable differences compared with preoperative Cooney scores(P〈0.05).According to Cooney standard,the results were excellent in 8 cases,good in 4 cases,fair in 2 cases,and the excellent and good rate was 85.71%.Conclusion Wrist arthroscopic technique is safe and effective in the treatments of Palmer typeⅠTFCC injuries.
