目的研究活动期肺结核病人和结核分枝杆菌ElISpot阳性健康体检者(结核分枝杆菌潜伏感染者)外周血单个核细胞(PBMCs)经结核分枝杆菌特异性抗原肽(ESAT-6,CFP-10)刺激后Th1、Th2、Th17和Treg细胞代表性细胞因子IFN-y、TNF—α、IL-6、IL-10、IL-17、TGF—β的表达特点及意义。方法24例初治活动期肺结核患者和25位结核分枝杆菌潜伏感染者,分离外周血单个核细胞(PBMCs),用ESAT-6,CFP-10肽库刺激PBMCs,采用FlowCytomix流式技术检测细胞培养液上清中细胞因子IFN-y、TNF—α、IL-6、IL-10、IL-17、TGF—β的表达。结果PBMCs经结核分枝杆菌特异性抗原肽刺激后,IL-17反应低下,而IFN-y、TNF-α、IL-6、IL-10和TGF—β均因刺激而升高,但只有IFN-y和TNF-α的水平在2组中具有显著性差异(P=O.013,P=0.006)。结论Th1细胞因子可能在结核病免疫病理中发挥更大的作用。
To well understand the immunopathology of tuberculosis, we evaluated the difference of cytokines induced by Mtb-specific antigen between active tuberculosis and latent tuberculosis infection in this study. Total of 24 patients with active pulmonary tuberculosis and 25 healthy controls with positive ELISpot were enrolled. The levels of IFN-γ TNF-α, IL-6, IL-10, IL-17 and TGF-β in supernatants of PBMCs stimulated with ESAT-6 and CFP-10 were analyzed with Bender Flowcytomix on flow eytometry. Except for IL-17, the cytokines were all increased, but only IFN-γ and TNF-α significantly increased in active pulmonary TB group as compared with control group (P= 0.013, P=0.006 separately). From all the result, we concluded that Th1 cytokines may play a greater role in the pathological progress of tuberculosis.
