【目的】观察电针治疗对大鼠脊髓损伤(SCI)后髓鞘变化及caspase-3、calpastatin表达的影响。【方法】选取雄性SD大鼠,随机分为模型组,电针组(电针大椎、命门穴),甲基强的松龙组(西药组)及空白组4组;除空白组外,其他3组大鼠均采用改良的Allen’s打击装置(50 g/cm)复制大鼠T10~T11脊髓中度损伤模型。分别于SCI后6 h、1 d、7 d取损伤局部脊髓组织,经半定量逆转录—聚合酶链式反应(RT-PCR)法和透射电子显微镜检查,观察各组脊髓损伤早期和继发期的caspase-3、calpastatin表达及髓鞘的病理变化。【结果】(1)电针组和西药组均能显著降低脊髓损伤6 h、1 d、7 d后脊髓组织中caspase-3 mRNA表达,升高calpastatin mRNA表达,与模型组比较差异均有显著性意义(P〈0.01);西药组与电针组比较差异无显著性意义(P〉0.05)。(2)模型组部分髓鞘变性,板层分离或消失,并与轴索分离;部分神经组织溶解,被坏死物质所代替。电针组髓鞘致密,轴索内可见线粒体,但仍可见部分髓鞘板层分离、变性。西药组部分髓鞘变性,板层分离;部分轴突水肿、凝固变性,部分轴索内可见线粒体。【结论】电针可降低脊髓组织中caspase-3 mRNA表达,升高calpastatin mRNA表达,从而抑制细胞凋亡,减缓SCI后病理损害,保护神经细胞,修复损伤脊髓,其作用与甲基强的松龙相仿
Objective To observe the mRNA expression of caspase-3 and calpastatin and on the changes of cell myelin sheath in rats after spinal cord injury(SCI) treated with electroacupuncture(EA).Methods SD male rats were randomized into the model group,EA group(EA on acupoints of Dazhui and Mingmen),methylprednisolone group and the blank group.Except for the blank group,rats in other groups were put into the modified Allen’s apparatus(50 g/cm) to induce middle-degree T10-T11 spinal cord injury.The injured local spinal cord tissue was sampled 6 hours,one day and 7 days after SCI.mRNA expression of caspase-3 and calpastatin in spinal cord was observed with semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR),and the early and secondary pathological changes of cell myelin sheath were examined under transmission electron microscope(TEM) in all groups after SCI.Results mRNA expression of caspase-3 was decreased and that of calpastatin in spinal cord was increased in EA group and methylprednisolone group 6 hours,one day and 7 days after SCI(P〈0.01 compared with those in the model group),and the difference was insignificant between EA group and methylprednisolone group(P〉0.05).In the model group,some cell myelin sheath became degenerated,the layers were separated or disappeared,and isolated from neuraxis,and some nervous tissue dissolved and was replaced by necrotic mass.In EA group,cell myelin sheath was dense,the mitochondrion appeared in neuraxis,and the separation and degeneration could still be found in some myelin sheath.In methylprednisolone group,some cell myelin sheath became degenerated,the layers were separated,some neuraxis became swollen,congealed or degenerated,and mitochondrion was found in some neuraxis.Conclusion EA has an effect on decreasing caspase-3 mRNA expression and increasing calpastatin mRNA expression in spinal cord,thus inhibiting apoptosis,relieving pathological changes,protecting neurons,and repairing injured spinal cord after SCI.The effect
