目的探讨细胞间黏附分子1(ICAM-1)在桥本甲状腺炎(HT)中的表达及其意义。方法选取2008年1月至2012年12月在山东大学附属省立医院甲状腺外科行手术切除的甲状腺疾病患者147例,根据疾病和肿瘤生物学行为不同分成HT组(72例)、甲状腺微小乳头状癌(PTMC)组(60例)、甲状腺腺瘤组(15例)。采用免疫组织化学法检测3组患者ICAM-1的表达。结果 ICAM-1在HT组(30.1%,22/72)及PTMC组(85.0%,51/60)中高表达,在甲状腺腺瘤组中无表达。ICAM-1在伴有甲状腺乳头状癌(PTC)样核特征的HT(61.9%,13/21)中表达显著高于不伴有者(17.6%,9/51),差异有统计学意义(P〈0.01)。不同年龄、性别、多灶性、有无复发和远处转移的ICAM-1阳性率比较差异无统计学意义(P〉0.05),有无包膜侵犯和淋巴结转移的ICAM-1阳性率比较差异有统计学意义(P〈0.05)。结论 ICAM-1在PTMC和伴有PTC样核特征的HT中表达上调,提示其可能参与甲状腺癌的发生、发展过程。
Objective To investigate the expression and clinical significance of intercellular adhesion molecule-1 ( ICAM-1 ) in Hashimoto thyroiditis (HT). Methods A total of 147 patients with thyroid disease admitted to Shandong University Affiliated Provincial Hospital Thyroid Surgery from Jan. 2008 to Dec. 2012 were included in the study, according to the disease and biological behavior of the tumor they were divided into HT group( 72 cases ) , small papillary thyroid carcinoma ( PTMC ) group ( 60 cases ) , thyroid adenoma group( 15 cases). The expression of ICAM-1 in the 3 groups was detected by immunohistochemistry(IHC). Results ICAM-1 expression was not detected in follicular lesions adenoma, but was frequently overexpressed in HT( 30. 1%, 22/72 ) and papillary thyroid microcarcinomas ( 85.0%, 51/60 ). ICAM-1 expression in patients with PTC-like nuclear aherations(61.9% ,13/21 ) was significantly higher than that in HT casesw- ithout PTC-like features( 17, 6% , 9/51 ) (P 〈 0.01 ). Differences of ICAM-1 positive rate among different ages,genders,multifocalness, recurrence and distant metastasis had no statistical significance (P 〉 0. 05 ). ICAM-1 positive rate between patients with or without capsular invasion and lymph node metastasis had statisti- cally significant difference( P 〈 0. 05 ). Conclusion ICAM-1 expression is upregulated in PTMC and HT with PTC-like features ,indicating that it might be involved in the genesis and development of thyroid carcinoma.
