目的观察嗜碱性粒细胞激发试验(BAT)的临床应用情况,为过敏疾病的诊断提供参考。方法选择10种吸人性过敏原对30例过敏性疾病患者行过敏原体外特异性BAT,福克全自动过敏原定量分析系统检测患者血清特异性IgE(sIgE)水平。对BAT与sIgE检测结果进行一致性检验。结果被检10种过敏原中,马上皮、链格孢属、枝孢属霉菌、艾蒿、白桦树及梯牧草BAT的阳性检出率明显高于血清sIgE检测,P均〈0.05。两种检测方法对过敏原检测的整体阳性率分别为23%、12.2%,P〈0.05;两种方法检测结果阳性吻合率最高的过敏原是屋尘螨和粉尘螨,分别为17%和13%,其余过敏原阳性吻合率均低于7%。两种检测方法对10种过敏原的检测结果一致性较差,Kappa均〈0.4。结论BAT对过敏原的检测范围比sIgE检测更宽泛,其不仅能帮助诊断sIgE阳性过敏,亦有可能对sIgE检测结果阴性即非IgE介导的过敏诊断有帮助。
Objective To assess the clinical application of basophil activation test (BAT) and to provide a reference for the diagnosis of allergic diseases. Methods Ten kinds of inhalational allergens were used to define the specific aller- gens of 30 patients with allergic diseases by BAT in vitro, and their allergen-specific IgE levels were detected by Fooke AL- LERG-O-LIQ system. The consistency between BAT and the sIgE test results was analyzed. Results In 10 kinds of aller- gens, horse epithelia, alternaria, cladosporium, mugwort, white birch and timothy showed higher positive detection rates of BAT than those of allergen specific IgE (sIgE) tests, all P 〈 0.05. The overall positive rates of the two detection methods were 23% and 12.2%, respectively, P 〈0.05. The results of the two methods were highly consistent with the allergen of dermatophagoides pteronyssinus and dermatophagoides farinae, which were 17% and 13 %, respectively. The positive rate in the rest of allergens was lower than 7%. There was poor consistent test results of two methods for the detection of 10 kinds of allergens, all Kappa 〈 0.4. Conclusions The detection range of BAT for allergen is broader than that of sIgE detection. And the BAT can not only help the diagnosis of sIgE positive allergy, but also give help to the diagnosis of sIgE negative test results which are non-IgE-mediated allergy.
