目的 研究血清IL-8及IL-18水平在骨肉瘤患者与健康体检者之间的差异及临床意义。方法 采用酶联免疫吸附法(ELISA)检测160例骨肉瘤患者及250例健康体检者的外周血血清IL-8及IL-18水平,比较两组间IL-8、IL-18水平的差异及它们之间的相关性,探讨血清IL-8及IL-18水平与骨肉瘤发病及进展的关系。结果 骨肉瘤患者组血清IL-8水平较健康对照组升高(P〈0.01),血清IL-18水平较健康对照组降低(P〈0.01)。在骨肉瘤患者组中,未发生远行转移的患者其血清IL-8水平低于已发生转移患者(P〈0.05),未发生远行转移的患者其血清IL-18水平高于已发生转移患者(P〈0.05)。AJCC分期中Ⅰ-Ⅱ期的低度恶性肿瘤相对于Ⅲ期同一骨断不连续的肿瘤、Ⅳ期伴有血源性肺转移以及远行转移肿瘤其血清IL-8水平均较低(P〈0.01),Ⅲ期较Ⅳ期伴有血源性肺转移以及远行转移肿瘤其血清IL-8水平略高,但差异无统计学意义(P〉0.05);Ⅰ-Ⅱ期患者较Ⅲ、Ⅳ期患者血清IL-18水平均较高(P〈0.01),Ⅲ期较Ⅳ期伴有血源性肺转移以及远行转移肿瘤其血清IL-18水平较高(P〈0.05)。结论 血清IL-8水平升高、IL-18水平下降可能与骨肉瘤发生、进展、转移有关,其作用可能表现为促进骨肉瘤细胞的增生和转移,两者有可能成为判断骨肉瘤的进展和预后的指标。
Objective To investigate the difference of serum levels of interleukin-8 ( IL-8) and interleukin-18 ( IL-18 ) between osteosarcoma patients and healthy controls and its clinical significance.Methods The peripheral blood serum levels of IL-8 and IL-18 in 160 patients with osteosarcoma and 250 healthy controls were detected by Enzyme-linked immunosorbent assay ( ELISA) method.The difference and relation serum levels of IL-8 and IL-18 in these two groups were compared, and the relationship between serum levels of IL-8 and IL-18 and the pathogenesis and progression of osteosarcoma were explored. Results Serum levels of IL-8 in the osteosarcoma group significantly increased(P〈0.01) ,but the levels of IL-18 obviously decreased while comparing with those of the control group(P〈0.01) .In osteosarcoma group, the serum levels of IL-8 in patients without metastasis were lower than patients with metastasis ( P〈 0.05) , while serum levels of IL-18 in patients without metastasis were highei,( P 〈 0 .05) .With respect to the AJCC staging,the serum levels of IL-8 of I ~ II low-grade malignant tumor were lower than that of stage with a bone broken discontinuous tumor, stage IV associated with hematogenous lung me-tastasis and metastasis tumor( P〈0.01) .Serum levels of IL-8 of stage IH were slightly higher than those of stage IV, but the difference was not statistically significant( P〉0.05) .Serum levels of IL-8 of stage I ?II were higher than those of stage IHand IV ( P〈 0 .01) .Serum levels of IL-8 stage IH with hematogenous metastasis of lung and tumor metastasis were higher than those of stage IV ( P 〈 0 .05) .Conclusion Increase of serum levels of IL-8 and decrease of serum levels of IL-18 may involve in the occun,ence,development and metastasis of the osteosarcoma,which may promote cell proliferation and metastasis.Serum levels of IL-8 and IL-18 can be used as a biomarker for detennining progress and prognosis of osteosarcoma
