目的探究Oculus眼表综合分析仪在干眼诊断临床应用中的重要作用。方法横断面研究。对2014年4~9月在南京医科大学附属眼科医院进行Oculus眼表综合分析仪检查的患者225例,男73例,女152例,年龄(39.2 ±21.1)岁。收集年龄、性别、非侵入检查第一次泪膜破裂时间(nonin-vasive first break-up time,NIKf-BUT)、平均泪膜破裂时间(noninvasive average break-up time,NIKav-BUT)、泪河高度,并进行了睑板腺拍摄、眼红分析、脂质层观察。将影响干眼的可能因素年龄、性别等分别与NIKf-BUT、NIKav-BUT进行相关性分析。结果在确诊的189例干眼患者中,性别与NIKf-BUT无相关性,统计值P =0.568;与NIKav-BUT无相关性(P =0.910)。年龄与NIKf-BUT呈负相关(P=0.000),直线回归方程为为=11.145-0.068X。年龄与NIKav-BUT呈负相关(P=0.000),直线回归方程为=14.460-0.070X。36例水液缺乏型干眼患者年龄与NIKf-BUT无相关性(P=0.964);与NIKav-BUT无相关性(P=0.702)。87例蒸发过强型干眼患者年龄与NIKf-BUT呈负相关(P =0.001);与NIKav-BUT呈负相关(P=0.003)。66例混合型的干眼患者年龄与NIKf-BUT呈负相关(P=0.004);与NIKav-BUT呈负相关(P=0.007)。结论结果显示性别不是干眼的影响因素,年龄是干眼的重要影响因素,尤其蒸发过强型干眼与年龄密切相关。Oculus眼表综合分析仪不仅可以诊断干眼,还可以分辨干眼类型,为干眼的对症治疗提供了临床依据。
Objective To investigate the application of the diagnosis in dry eye using a newly developed oculus keratograph. Methods A total of 225 patients taking Oculus keratograph with a mean age of 39.2±21.1 years were gathered in retrospective study. Some parameters including age, gender, NIKf-BUT, NIKav-BUT, tear meniscus height were collected. And meibo-scan, bulbar redness, lipid layer examination were taken. The correlations between the NIKf-BUT, NIKav-BUT and possible influencing factor such as age, gender were determined. Results In 189 dry eye patients, no significant correlation was detected between the NIKf-BUT (P =0.568), NIKav-BUT (P =0.910) and gender. A significant difference between age and the NIKf-BUT (P =0.000) was found, and linear regression equation was Y=11.145-0.068X. A significant difference between age and the NI-Kav-BUT (P =0.000) was found, and linear regression equation was Y=14.460-0.070X. Taking further analysis of dry eye parting, in 36 dry eye patients of lacking water type, no statistically significant difference was observed between the age and the NIKf-BUT (P =0.964, NIKav-BUT (P = 0.702). However, there was significant difference between the age and the NIKf-BUT (P =0.001), NI- Kav-BUT (P =0.003) in 87 evaporative dry eye patients. And there was significant difference between the age and the NIKf-BUT (P =0.004), NIKav-BUT (P =0.007) in 66 mixed types. Conclusions Our results show that age is one of the important factors affecting dry eye occurring, but gender is not. Especially strong evaporation type dry eye associates with age closely. The older you get, the more prone to dry eye. Oculus keratograph can not only diagnose dry eye, but also distin- guish the types of dry eye, which provides the clinical basis for symptomatic treatment of dry eye.
