目的:对兔采用自体骨、异体骨及组织工程骨移植进行腰椎后外侧横突间融合手术,比较分析MRI、CT、X线3种影像手段在早期骨移植中的诊断价值。方法:分别在30只健康新西兰大白兔的双侧腰椎横突间植骨并制备模型,依据填充的材料不同随机分为三组。A组:组织工程骨材料;B组:自体移植骨材料;C组:异体移植骨材料。术后2、4、6周行骨密度检测,CT及MRI检测。结果:X线及CT显示术后6周用组织工程骨作为移植材料的骨密度值、横突间融合率明显高于自体骨及异体骨。MR动态增强扫描(DCE-MRI)能早期无创监测移植骨新骨形成的骨基质中原始微血管的形成过程,判断移植骨存活情况。结论:MRI、CT、X线联合应用能监测兔腰椎融合术后的移植骨修复过程。动态增强MRI对早期骨移植是否成功有很好预测作用。
Purpose:Posterolateral intertransverse process fusion was performed in rabbit lumbar spine using the autogenous iliac,the allogeneic bone and the tissue-engineered bone.Diagnostic value of MRI,CT,radiography was evaluated at the early phase of bone transplantation.Methods:Thirty healthy New Zealand white rabbits with bilateral lumbar intertransverse bone grafting were used as models for comparision in our study.The rabbits were divided into 3 groups according to the materials used for defect filling,A: the tissue-engineered bone graft group,B: the autogenous iliac group,C: the allogeneic bone graft group.Gross inspection with bone density testing,CT and MRI were done at 2 weeks,4weeks,and 6 weeks.Results: Six weeks after surgery,radiography and computed tomography indicated that the bone density and posterolateral fusion rate of tissue-engineered bone graft group was obviously higher than other two groups.DCE-MRI was an noninvasive method,which can early monitor primitive microvascular forming process in bone stroma during allograft new bone formation,and determine the graft survive.Conclusion: Combined with MRI,CT and radiography,the graft healing process of the rabbit lumbar spinal fusion surgery can be monitered.DCE-MRI has a good prediction value on early success of bone transplantation.
