目的观察益心舒胶囊联合米氮平对恢复期卒中后抑郁(PSD)气阴两虚兼血瘀型患者抑郁及神经功能恢复的疗效,及对血中5-羟色胺、去甲肾上腺素的影响。方法把符合中西医诊断及纳入标准的患者随机分为治疗组和对照组,每组各60例。治疗组予益心舒胶囊联合米氮平口服,对照组仅予米氮平,疗程均为8周。采用HAMD减分率评价抑郁疗效,SSS及Barthelk~数(BI)评价患者神经功能及日常生活能力改善情况,同时治疗前后检测两组患者血中5.羟色胺、去甲肾上腺素的水平。结果治疗8周后,治疗组HAMD减分率优于对照组(P〈0.05);两级SSS评分在治疗8周时都有下降,但治疗组改善情况优于对照组(P〈0.05),且较对照组改善更快;两组BI评分在治疗8周后都明显提高,但治疗纽提高优于对照组(P〈0.05),且较对照组提高更快;两组治疗后血浆5-HT、NE含量均较治疗前有所升高,但前后比较无显著性差异,两组治疗后比较无显著性差异。结论益心舒胶囊联合米氮平能明显改善PSD患者抑郁症状,有利于神经功能恢复及日常生活能力的改善。
Objective To observe the Yixinshu capsule combined with mirtazapine on recovery of post-stroke depression (PSD) deficiency of both qi and Yin and blood stasis in patients with depression and recovery of neurological function effect, effect of 5- serotonin, norepinephrine and to study the blood. Methods To comply with Chinese and Western medicine diagnosis and the inclusion criteria were randomly divided into treatment group and control group, each group had 60 cases. The treatment group received Yixinshu capsule combined with Mi Danping orally, the control group only received Mi Danping, treatment for 8 weeks. The reduction rate of AMD depression curative effect evaluation, SSS and Batthel index (BI) in patients with neurological function and daily living ability evaluation to improve the situation, at the same time before and after the treatment, the levels of 5- serotonin, norepinephrine and two groups of patients. Results After 8 weeks of treatment, the treatment group reduced rate of HAMD was better than that of the control group (/'〈0.05); the two group the SSS score at 8 weeks of therapy has decreased, but the treatment group improved better than the control group (P〈0.05), and compared with the control group improved faster; BI scores of the two groups at 8 weeks after treatment were significantly improved, but the the treatment group improved better than the control group (P〈0.05). Conclusion Yixinshu capsule combined with mirtazapine can significantly improve the PSD patients with depression, is conducive to the recovery of neurological function and daily life ability improvement.
