目的探讨基本药物制度实施对基层医疗卫生机构运行的影响。方法选取浙江、江西、陕西和新疆四省(自治区)6城市共31家基层医疗卫生机构,对机构开展问卷调查,了解基层医疗卫生机构卫生服务业务量和费用等情况。结果与基本药物制度实施前相比,样本地区基层医疗卫生机构的总体平均门急诊人次上升,大部分机构的次均门诊费用和次均门诊药品费用下降,与2009年相比,2012年A、B、C三地的次均门诊费用较分别下降30.3%、25.6%和43.3%。结论基本药物制度的实施使基层医疗机构门诊服务提升量提升,次均门诊费用降低,取得了一定的效果。
Objective To understand the effect of the ‘Essential Medicines Scheme'on the operations of the county level medical institutions in China. Method A total of 31 primary health institutions from Zhejiang,Jiangxi,Shaanxi and Urumqi were sampled as the subjects of study in the form of a questionnaire survey. The volume of business and the cost in the institutions to be surveyed were the focus of the investigation. Findings Compared with the implementation of the basic drug system,the average number of outpatient and emergency visits were all higher than before 2009. The average costs and the average drug costs of outpatients dropped in some institutions. The average costs and the average drug costs of outpatients dropped in three regions( labeled as A,B and C) of the four provinces by 30. 3% 、25. 6% and 43. 3% respectively.Conclusion The implementation of the‘Essential Medicines Scheme'promotes the service of the medical institutions. The effect of the scheme is particularly manifested in the drop of clinical costs and hence has some positive implication.
