目的探讨伴儿童期忽略抑郁症患者前扣带回氢质子波谱物质代谢的特点。方法采用儿童期创伤问卷(childhood trauma questionnaire,CTQ)对40例抑郁症患者进行儿童期忽略评估和分组,伴儿童期忽略抑郁症组19例患者和不伴儿童期忽略抑郁症组21例患者,以及20名正常对照行磁共振氢质子波谱(hydrogen magnetic reso-nance spectroscopy,^1 H—MRS)扫描,兴趣区选取双侧前扣带回(anterior cingulate cortex,ACC),检测N-乙酰天门冬氨酸(N—acetyl aspartate,NAA)、谷氨酸复合物(glutamate/glutamine,Glx)、胆碱(choline,Cho)、肌醇(myo—inositol,mI)及肌酸(creatine,Cr)水平,比较3组NAA/Cr、Glx/Cr、Cho/Cr和mI/cr比值的差异。结果伴与不伴儿童期忽略抑郁症组分别与对照组比较,左右两侧ACC均表现NAA/Cr降低(均P〈0.010);2组右侧Glx/Cr均低于对照组(均P〈0.001);伴儿童期忽略抑郁症组较不伴儿童期忽略抑郁症组左右两侧NAA/Cr差异均有统计学意义(左P〈0.005,右P〈0.01)。结论抑郁症患者前扣带回物质代谢不同于正常人;伴儿童期忽略抑郁症患者ACC的物质代谢存在特异性改变。
Objective To investigate the changes of metabolism in the anterior cingulate gyrus of depressive pa- tients with childhood neglect by ^1H-Magnetic resonance spectroscope (IH-MRS). Methods Forty depressive patients were divided into two groups (19 depressive patients with childhood neglect and 21 depressive patients without childhood neglect) by the Childhood Trauma Questionnaire (CTQ). All the patients and 20 healthy controls were scaned using the hydrogen magnetic resonance spectroscopy (^1H-MRS). The data of N-acetyl aspartic acid (NAA), choline (Cho), creatine (Cr), glutamate/glutamine (Glx) and inositol (mI) in bilateral anterior cingulate were collected. NAA/Cr, Glx/Cr, Cho/Cr, mI/Cr were then calculated and compared among the three groups. Results Compared with the control group, the depres- sive patients with/without childhood neglect showed decrease of the bilateral NAA/Cr (all P〈0.O1) and right Glx/Cr (all P〈 0.001). NAA/Cr was significantly different between depressive group with childhood neglect and one without childhood neglect (left P〈0.005; right P〈0.01). Conclusion Patients with depression show metabolic abnormalities in the bilateral ACC. There may be an unique feature of ACC metabolism in depressive patients with childhood neglect.
